Southwest Medical, part of OptumCare, is a multi-specialty group of physicians, Nurse Practitioners, and Physician Assistants consisting of over 300 providers, 21 medical health centers including six urgent care clinics, four convenient care centers, two lifestyle centers catering to older adults, an outpatient surgery center, home health, hospice and palliative care services, plus E-visits and online options via Now Clinic telemedicine technology. Southwest Medical offers patients compassion, innovation, and quality care throughout southern Nevada. Southwest Medical is headquartered in the greater Las Vegas, NV area.

The LPN is responsible for processing home health referrals. Must have experience working within a team environment, possess excellent organizational skills, with the ability to multi-task. Strong telephonic communication skills required. Previous Home Health experience preferred.

Primary Responsibilities:

Telephonic in-take and processing of home health referrals
Communicate and collaborate with other members of the health care team (PCP, CMA, health plan) to coordinate all needed care andservice
Take verbal and written orders from providers
Write and code orders to be electronically faxed

Required Qualifications:

High School diploma and / or equivalent

Current, unrestricted NV LPN license or temporary NV LPN license with the ability to obtain permanent NV LPN license upon expiration of temporary NV LPN license
2+ years clinical experience
1+years of working with all Windows PC applications in a work environment
1+ years experience charting in EMR (Electronic Medical Records)
Preferred Qualifications:

Knowledge of CMS guidelines for State of Nevada and/or prior home health experience
1 year of case management experience
Bi-lingual or multi-lingual
