General Summary / Purpose

The Medical Office Coordinator (MOC), or Care Transitions Coordinator (CTC) on CIMS, will be responsible for providing administrative support to providers on the Medicine service at Johns Hopkins Bayview Medical Center. This role will interact with patients, patients' families, referring physicians and third-party carriers with regard to patient-related issues. The Coordinator will work with providers to carry out the administrative tasks necessary to ensure smooth operation with care coordination and maintain good patient relations.

Duties and Responsibilities

Communication:

External:





  • Contact patient's primary care provider (PCP) office to:





  • Inform them of patient's admission and covering provider





  • Schedule phone meetings for PCP and covering providers to update on patient status (serve as a warm handoff between the providers).





  • Obtain patient medical records.





  • Call pharmacy to obtain patient medication lists for medication history and reconciliation processes if there is no documentation prior to patient arrival on the Medicine unit.





Internal:





  • Assist with the coordination of collaborative patient rounding for the doctors and nurses.





  • Coordinate with case management and social work and then notify nurses that the discharge process is complete and remaining orders are placed.





  • Coordinate with case management to follow up on scheduled tests and procedures.





Operational Tasks:





  • Support the physician with insurance preauthorization by submitting necessary clinical information as instructed by the physician.





  • Partner with providers to ensure timely completion of various tasks:





  • Confirm that pending inpatient discharge tasks are being completed (i.e. blood work, Echo, MRI, endoscopy, line placement, etc.).





  • Remind providers in monitoring of signing of daily notes and/or discharge summaries (for patients not going home) according to policy.





  • Pick up patient medications from pharmacy when a Meds for Home Coordinator is unavailable to deliver to the patient room.





  • Facilitate the scheduling of family meetings (i.e. Case management will identify family availability and MOC will determine provider's availability. Work with case management to determine meeting details (date, time, location).





  • Assist with administrative paperwork associated with coordination of patient transfers to other sites like ALFs, group homes, guardian ship paper work etc.





Administrative Tasks:





  • Responsible for faxing and receiving faxed documents.





  • Complete forms for patients (i.e. back to work, Assisted Living Facility forms/group homes).





  • Ensure that nurse phone number in Epic is accurate.





Other duties as assigned.

Qualifications

High School Diploma or GED required. Additional education may substitute for required experience, to the extent permitted by the JHU equivalency formula.

Salary Range: Minimum 16.86 per hour - Maximum 27.84 per hour. Compensation will be commensurate with equity and experience for roles of similar scope and responsibility.

In cases where the range is displayed as a $0 amount, salary discussions will occur during candidate screening calls, before any subsequent compensation discussion is held between the candidate and any hiring authority.
