Overview

The position of Jail Diversion Case Manager exists to assist clients in the areas of service referrals, collaborating with other resources to secure mental health services for jail inmates, and ensure inmates begin utilizing assigned services when released from jail.


Responsibilities

Meets with incarcerated individuals at county jails at request of law enforcement personnel; assists inmates in obtaining referrals to mental health services; assists mental health professionals in determining physical and mental health and substance use treatment needs of inmates.

Either through post-incarceration services or through referrals from County Mental Health Services, determines appropriate provider of services, assists client in applying for services, provides direct client services until client is able to begin services with the referral source, assists with the development of a client crisis plan, provides transportation to clients when needed to ensure client meets appointment requirements.

Coordinates referrals for pre-commitment screenings, contacts concerned others seeking mental health commitment for individual experiencing mental health issues; educates clients, families, and care advocates in wellness, medication needs, and health services; provides follow-up with client as needed.

Responds to calls from law enforcement personnel related to mental health issues, assists in the development of client crisis plans; refers individuals to appropriate mental health services; provides follow-up with client as needed

Reports required outcome information and assists in the preparation of monthly billing by providing documentation.

Attends community meetings and/or trainings at the request of the Supervisor.


Qualifications

Education: Bachelor's degree required. Preferred in human services, social services, or related field.

Experience: Two years' experience in human services field, preferably in a jail or re-entry capacity.

License(s)/Certification(s): Valid driver's license when driving any vehicle for work-related reasons. Must provide proof of automobile liability insurance meeting Eyerly Ball requirements, and a driving record meeting liability insurance carrier requirements.

Knowledge/Skills/Abilities: Extensive knowledge of community based services available to address client needs. Able to communicate with individuals undergoing life challenges due to mental health issues. Knowledge of and understanding of population and demographics of the communities served

Strong written and oral communication skills. Experience working with diverse clientele and organizations. Demonstrated ability to develop relationships through networking, team building, and collaborative skills. Self-directed and commitment to work schedule that requires flexibility and dedication to goal achievement. Able to work within the community with various leadership and law enforcement personnel

Other: Use of usual and customary equipment used to perform essential functions of the position.

Must have vehicle available for use in transporting clients
