Below, you'll find other important information about this position. Provides clinical leadership for ACT program. As a member of the treatment team(s) of the program, the lead is responsible for providing a broad range of treatment and case management functions for the population served. Provides clinical program coordination. Responsible for day-to-day operation of the program and work closely with Manager/Admin team to keep them informed of staffing related items, referrals, programming, staff performance, and quality and safety items. Coordinates utilization review activities for the purpose of monitoring standards of patient care and to take action to improve care and maintain quality care. Work is performed in accordance with established professional policy, procedures, practices, regulatory requirements and accrediting body standards. Provides professional nursing care to clients in the ACT Program. Directs and supervises activities and assignments of ancillary personnel to the ACT Program. Is responsible for providing a broad range of treatment and rehabilitation activities.

MINIMUM QUALIFICATIONS:

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

1. Current Registered Nurse license issued by the state in which services will be provided or current multi-state Registered Nurse license through the enhanced Nurse Licensure Compact (eNLC).

2. Obtain certification in Basic Life Support within 30 days of hire date.

3. Valid Driver's License.

4. State criminal background check and Federal (if applicable), as required for regulated areas

EXPERIENCE:

1. Three (3) years clinical experience in mental health field.

2. Two (2) years Supervisory experience.

PREFERRED QUALIFICATIONS:

EDUCATION, CERTIFICATION, AND/OR LICENSURE:

1. Advanced certification.

2. Bachelors of Science in Nursing (BSN) or higher.

CORE DUTIES AND RESPONSIBILITIES: The statements described here are intended to describe the general nature of work being performed by people assigned to this position. 1. Provides direct clinical supervision to all unit clinical staff in a specified program.

2. Assesses competency of clinical staff through observation and testing techniques.

3. Assists clinical staff in developing required competencies.

4. Gives input to Manager on performance management reviews of the unit staff.

5. Develops and maintains a dynamic clinical program based on best practice.

6. Participates as clinical personnel on a treatment unit and facilitates therapeutic milieu on the unit.

7. Bases decisions made/actions taken on relevant scientific principles, established standards of care and/or research findings.

8. Assess patient needs, including assessing for high-risk indicators and documents history/assessment in a manner that is clinically pertinent to the individual patient.

9. Participates as an integral professional member of an interdisciplinary treatment team led by a psychiatrist.

10. Provides direct clinical assessment and intervention services to individuals, groups and families.

11. Performs discharge planning as developed with the treatment team from day of admission as needed.

12. Participates in treatment planning sessions and assists treatment team by coordinating planning effort.

13. Acts as liaison for hospitalized patients and their families to various community agencies, hospital consultants and services.

14. Records and charts daily patient contacts as required by hospital policy.

15. Assists in quality reviews of program as assigned by the manager.

16. Assists in maintaining regulatory compliance of the program.

17. Maintains appropriate certification, licensure and competency through supervision and/or training and successful completion of annual performance evaluations.

18. Attends continuing education seminars and inservices as necessary to maintain professional licensure, competency, and satisfy hospital required education standards.

19. Provides training of staff on various issues and topics relevant to the specific patient population.

20. Participates in Performance Improvement activities as appropriate.
