Pre-Sentence Re-entry Navigator

Must hold at least a Master's Degree from an accredited graduate institution. Must have at least one year of experience working with clients in the criminal justice field.

Job Purpose:
To promote efficient coordination of client care for individuals with multiple, complex and/or chronic areas of need.

Pre-Sentence Re-entry Navigator




    1. Pre-Sentence Navigator will complete more thorough screening to identify clients with shorter length of stay and priority given to high utilizers (3+ arrests in past year).
    2. Facilitate open referral process with inmates where transitional resource packets are shared, reviewed, competed as well as offer Brief Intervention Therapy as needed. More intensive case management provided on an ongoing basis. At any time, a staff member, mental health clinicians, an outside agency, and/or inmates can refer to receive services.
    3. Works collaboratively with our Collaborative Programs for Reintegration (CPR) group and meets every Wednesday morning for staffings. They typically include the following:




  1. JBBS
  2. Local community providers (AllHealth Network)



  • Department of Human Services-Medicaid



  1. ARMOR- our jail medical vendor
  2. Community (Douglas County Cares program)
  3. Local employment workforce



  • Other CJ stakeholders (pre-trial, re-entry, probation, public defenders, jail staff)




    1. Coordinate services with local community behavioral health providers prior to release of an inmate to ensure continuity of care. Make referral appointments based upon need and provide the appointment date to individual before release, most likely coordinating ride to treatment with deputies.
    2. Create a transition plan for clients releasing from custody. Will provide ongoing and as-needed case management to clients until upon release.
    3. Data reporting in OBH database.



Key Technical Skills and Knowledge:
Education:

  • Masters' degree in a health/behavioral health-related area of focus such as social work, psychology, or sociology; or a Masters's degree in an unrelated field with at least one year of experience in the mental health field.
  • Experience:

  • Minimum of 2 years of related experience with diverse populations in a physical and/or behavioral health setting.
  • Experience in case management, care coordination, and/or community health required.
  • Skills/Knowledge:

  • Demonstrated ability for assessment, evaluation, and interpretation of behavioral health information, and care planning.
  • Knowledge of and familiarity with case management concepts, principles and strategies.
  • Understanding of community resources, treatment options, funding options and special programs.
  • Ability to operate PC-based software programs including proficiency in Word, Excel and PowerPoint and other computer based systems.
  • Advanced written and oral communication, decision making and problem solving skills, efficient time management with multi-tasking ability.
  • Excellent interpersonal skills, the ability to work independently and as part of a team; collaborating with other internal and external teams.
  • Must possess own transportation, have a good driving record, a valid driver's license, and proof of insurance.

    Salary: $50,000 - $54,000 annually

    The base salary range represents the low and high end of the AllHealth Network salary range for this position. Actual salaries will vary and may be above or below the range based on various factors including but not limited to experience, education, training, merit, and the ability to embody the AllHealth Network mission and values. The range listed is just one component of AllHealth Networks' total compensation package for employees. Other rewards may include short-term and long-term incentives as well as a generous benefits package detailed below.
