Care Navigator - Hospital Stepdown

Description

We are hiring for a Care Navigator to work in our Client Access team. This is a full-time position, working 40 hours a week between Monday - Friday, 8-5PM. This individual will be working with hospital step-down patients, to increase engagement for those stepping down from a higher level of care. This person is responsible for connecting them with outpatient services with AllHealth Network. This is a community-based role, meeting clients where they are in the community, making face-to-face relationships with hospital partners in the community, and decreasing barriers to care that will impact the continued need for a higher level of care.

Hourly Rate: $20 - $23.61 an hour

Goals and Objectives:



  • Engaging high risk clients in the level of care appropriate to meet recovery goals.
  • Communicate with clients, community and internal partners, and the team about needs, action plans and next steps.
  • Provide needed case management to ensure clients have what they need to successfully engage in care.


Duties and Responsibilities:



  • Extract Excel based data reports from the electronic health record on a weekly basis.
  • Organize, filter and develop tracking mechanism for interventions around caseload management.
  • Communicate with Central Access, Clinicians, Medical Providers, Supervisors and Managers regarding status of caseloads and client needs.
  • Based on the direction of the clinical team and/or AllHealth Network policies and procedures, complete administrative update and closing of client charts.
  • Schedule and cancel client appointments based case management reports and client communication.
  • Participate in telephonic communication with clients regarding their engagement in treatment. Complete and manage written communication with clients regarding patient status for receiving care at AllHealth Network. Assist with care coordination tasks such as appointment monitoring and facilitating communication with providers and other care team members.
  • Document all interventions in the electronic health record.
  • Perform administrative support for programs including but not limited to: medical record/chart review, documentation of patient information, review and assignment of cases to clinical staff.


  • Participate and contribute to departmental meetings, supervision and other meetings, as needed, to ensure cohesive department operations.
  • Enhance professional growth and development through participation in specialized training and other informal learning experiences.
  • Follow all AllHealth Network policies and procedures.
  • Complete all required trainings as listed in Relias Learning within required timelines.


Perform other duties as required within the scope of the position and the experience, education and ability of the employee.

Key Technical Skills and Knowledge:
Education:



  • Requires Bachelor in Social or Human Services field



Experience:



  • One year of experience in a customer service or healthcare-related field such as a hospital, home health provider, or community-based agency preferred but not required.



Skills/Knowledge:



  • Possess excellent customer service skills, strong organization skills, flexibility in responding to multiple demands, able to complete routine work with minimal direction from supervisor.
  • Ability to operate PC-based software programs including high level proficiency in Word, Excel and other computer based systems.
  • Understanding or a willingness to learn care coordination and case management related service functions (i.e.: outreach and engagement, community resources, discharge planning, referrals, insurance/payer).
  • Communicate effectively in written and oral communication with health care team, clients and families.

The base compensation range represents the low and high end of the AllHealth Network compensation range for this position. Actual compensation may vary and may be above or below the range based on various factors including but not limited to experience, education, training, merit, and the ability to embody the AllHealth Network mission and values. The wages provided here are just one component of AllHealth Networks' total compensation package for employees. Other rewards may include short-term and long-term incentives as well as a generous benefits package as outlined below.
