This is an excellent opportunity for a Centralized Follow-Up Specialist (40hrs/wk., benefits eligible).
PURPOSE of POSITION:
Under the direction of the Director of Centralized Follow-Up (CFU) and Care Coordination, the CFU and Care Coordination Specialist will be responsible for providing medical follow-up to patients as well as assisting licensed staff with CFU processes.

CPR certifiable.
EDUCATION and/or EXPERIENCE:
High school diploma or equivalent. Two years of experience in healthcare or social service setting is required. Experience in family planning or an abortion setting strongly preferred.
PHYSICAL REQUIREMENTS:

Physical demands of the position necessitate hearing, talking, handling and grasping regularly. May require lifting up to 30 pounds. Specific vision conditions include close vision reading. Requires the ability to operate a computer, telephone, fax, copy machine and adding machine or calculator. Requires the ability to operate medical equipment.
WORKING CONDITIONS:
The position is generally performed in a medical office environment but involves frequent interruptions. The noise level is generally low to quiet. The work environment characteristics are representative of those an employee encounters while performing the essential functions of this position. Reasonable accommodations, however, may be made to enable individuals with disabilities to perform said essential functions.
Work environment involves exposure to potentially dangerous materials and situations that require following extensive safety precautions and may include the use of protective equipment. Will work with blood or blood-borne pathogens and will require OSHA training.
Must have reliable transportation. This position requires occasional traveling to Milwaukee, therefore a valid driver's license and auto insurance in accordance with agency liability standards will be required.
Evening and/or weekend hours are occasionally necessary.
Essential Duties and Responsibilities:
  • Provide medically accurate education for all laboratory & medical results as assigned in the CFU and care coordination center and accurately document in the medical record/plan.
  • Accept and manage tasks, make second and third contact attempts within specified time frame for all centralized follow-up along with the necessary information sheets.
  • Run daily reports from electronic health records to determine next action steps in follow-up.
  • Maintain next action date tickler system in the electronic health system.
  • Make appropriate appointments in PPWI's practice management system for patients at all health centers for specific treatment and follow-up of abnormal lab results and in-house referrals.
  • Answer phones, check medical results voicemail throughout the day, return appropriate calls and document client information from calls.
  • Meet metrics for volume/time for reminder letters and contact attempts.
  • Enter necessary STI information into the State's WEDSS system within the necessary timeframes.
  • Scan all lab results as assigned into the electronic health record system.
  • Manage incoming and outgoing medical records requests.
  • Maintain ongoing and appropriate communication with all staff in the CFU & care coordination center as well as the patient services team.
  • Complete abstraction work as needed.
  • Assist with quality assurance activities as appropriate.
  • Perform clerical and/or administrative duties as assigned.
  • Complete other duties as assigned.
