Program Manager- Ambulatory Team Health Initiatives- Days

The Program Manager Team Health Initiatives will support the development, planning, execution and ongoing monitoring by working in collaboration with Dept. leadership and Ambulatory operations to develop and provide admin oversight for an employed and dependent focused episodic-care delivery model, including the first team member health clinic across the VCU Health System.

Essential Job Statements

Leverage technology and industry best practice to create a high-technology, high-touch health care delivery approach focused on timely and easy access, seamless coordination of care across the system, and emphasis on providing the right support at the right time to improve the lives of team members and their families.

Collaborate with VCU primary care departments, Strategic Planning, Real Estate, Ambulatory and senior leaders to analyze market demand and zip codes of team member and their dependents to determine phased-in site selection, lease/purchase arrangements, provider recruitment and network planning and clinic operations for future sites. Assist in Developing a strategy to expand VCU Health Primary Care.

Collaborate with specialty departments to streamline access to specialty appointments and coordinate care reducing no show rates, improving access and overall health outcomes. In collaboration with IT, develop an online tracking mechanism to identify the status of referral requests and identify an SLA with specialty departments to provide consultation in a timely fashion.

Collaborate with Marketing and Communications to develop a marketing and communication plan for the initial launch and future expansion. Work directly with third party payor to streamline employee benefit communications and to focus on ongoing patient education to improve overall health outcomes.

Develop a patient concierge model that streamlines employee and dependent access to primary care using industry best practice, technology, and high-touch customer service. Analyze local competitors and consider partnerships and affiliations, if appropriate, to meet the demand.

Partner with Benefits COE to monitor the annual EE Plan medial spend. Supports review for additional savings.

Collaborate with VCU Health Virtual Care Institute to leverage telehealth and incorporate as an essential component of Team Health. Leverage innovation and collaborate to promote workplace wellness and engagement.

Serve as internal and external subject matter expert regarding Team Health.

Collaborate with senior leadership team to determine applicability of expanding DTE model and/or developing additional onsite or telehealth options.

Required Education: Master's degree in business management, Finance, Accounting, Healthcare, Administration or Nursing from accredited program.

Minimum Qualifications

Years and Type of Required Experience: Minimum 5-7-year professional experience to include progressive supervisory experience, preferably in a health care setting. Experience Preferred: Experience with Access, clinical workflows, Lean Six Sigma, project coordination.
