Under general supervision of the Transplant Social Work Manager, candidate is responsible for ongoing pre-transplant psychosocial assessment, waitlist management and post-transplant follow-up of Abdominal Transplant and Cardiothoracic Transplant patients, as well as living donor/recipient assessments as needed. Candidate will facilitate transplant and caregiver support groups and participate in various transplant committees. Responsible for providing resources for local housing for out-of-town patients/families. Must assist in coordination of discharge planning as needed and collaborate with transplant staff and social workers in other transplant programs. Position includes crisis intervention, mandated reporting, information and referral coordination of health and mental health services with community providers. The social worker is responsible for education and advocacy of patients, families and/or significant others. Candidate will participate in related department functions, such as community and program education. Collaborate with the transplant team and social workers in other transplant programs. Depending on the experience of the selected candidate, this position will be hired at a Clinical Social Worker II or Clinical Social Worker III level.
Pay Transparency Act Annual Full Pay Range: $85,232 - $112,293 (will be prorated if the appointment percentage is less than 100%) Hourly Equivalent: $40.82 - $53.78 Factors in determining the appropriate compensation for a role include experience, skills, knowledge, abilities, education, licensure and certifications, and other business and organizational needs. The Hiring Pay Scale referenced in the job posting is the budgeted salary or hourly range that the University reasonably expects to pay for this position. The Annual Full Pay Range may be broader than what the University anticipates to pay for this position, based on internal equity, budget, and collective bargaining agreements (when applicable). |
