Summary

  • The registered nurse in Home Care assists in all facets of the referral process (preadmission, admission, follow-p and discharge). The registered nurse assesses, plans, implements, evaluates, and supervises individual patient care following established standards of care. Accepts assignments and travels to homes to deliver care. The Registered Nurse provides on-call coverage during non-business hours of operations.

Duties

  • Completes an initial assessment of patient and family to determine home care needs. Provides a complete physical assessment and history of current and previous illness/es. Documents all care timely.
  • Regularly re-evaluates patient nursing needs. Reports changes in patient condition to physician. Obtain/receives physician's orders as required.
  • Develops and Initiates the plan of care and makes necessary revisions as patient status and needs change.  Uses health assessment data and OASIS/HIS data to determine nursing diagnosis.                    
  • Initiates appropriate preventive and rehabilitative nursing procedures. Administers medications and treatments as prescribed by the physician.
  • Provides on-call coverage during non-business hours.
  • Provides health care instructions to the patient as appropriate per assessment and plan of care. Counsels the patient and family in meeting nursing and related needs.
  • Identifies discharge planning needs as part of the care plan development and implements prior to discharge of the patient.
  • Acts as Case Manager when assigned by Lead RN/Associate Team Leader and office/intake nurse and assumes responsibility to coordinate patient care for assigned caseload. Ensures that arrangements for equipment and other necessary items and services are available.
  • Prepares clinical notes and updates the primary physician when necessary and at least every sixty days. Communicates with the physician regarding the patient's needs and reports any changes in the patient's condition; obtains/receives physician's orders as required. Communicates with community health related persons to coordinate the care plan.
  • Instructs and supervises both LPN's and PCA's/PCT's as well as evaluates home health aide care provided every two (2) weeks.

Qualifications

  • Nursing Diploma or Degree from an accredited school of nursing, BSN preferred.
  • Current certification or licenses as required by state law within the practicing state. BLS certification required within 90 days from start date. If operating a motor vehicle for business purposes, a valid driver's license is required.
  • Minimum of one year acute care or public health experience. Home Care experience preferred.
