Overview
The Hospice Quality Review RN is responsbile for data collection, documentation review, and participation in Performance Improvement Projects (PIPs). This position serves as a resource to the Director of Quality and Compliance on all matters related to the quality of hospice care and services including development of training modules to address quality and documentation compliance concerns Displays an ongoing commitment to excellence in customer service to both internal and external customers. The Elizabeth Hospice is San Diego's oldest non-profit hospice of 45 years. We are growing and looking for talented individuals to join our high performing team and support our mission. Work Schedule: Monday through Friday, 8:00 a.m.-5:00 p.m. Salary: $45-61/hr Subject to experience and qualifications including years of licensure Work Location: Escondido, CA
Responsibilities
Assists in implementation of quality assessment monitors, questionnaires, and interview instruments.
- Participates in clinical reviews of all hospice care and services including Hospice IDG, Volunteer, Bereavement, Counseling, and Palliative Care services by utilizing both concurrent and retrospective studies to identify high risk and problem prone areas of care delivery.
- Regularly monitors the quality of all services provided under arrangement to include nursing facilities, pharmacy, DME, PT, OT, ST.
- Participates in ongoing monitoring through observations, interviews, and document review.
- Facilitates the implementation of performance improvement projects as designated by the Director of Quality and Compliance.
- Provides staff education related to quality of care and services as designated by the Director of Quality and Compliance or designee.
- May randomly attend IDG meetings for the purpose of monitoring the IDG process and identifying opportunities for improvement.
- Conducts field reviews of documentation at the site of care to assess compliance with inclusion of the patient, family, caregivers, and facility staff in implementing the patient's plan of care.
- Participates in internal (mock audits, focused reviews) and external quality of care and documentation compliance reviews by licensing or accreditation agencies.
- Utilizing both internal and external expertise and resources, provides staff education and presentations related to identified documentation compliance and/or quality of care concerns and documents such activity.
- Other duties as assigned.
Qualifications
- EMR experience required.
- Associate Degree in Nursing. Bachelor's Degree in Nursing preferred.
- Current RN license.
- Certification as hospice nurse preferred
- Documented experience in quality assessment and performance improvement in health care environment.
- Excellent communication skills, both verbal and written, problem solving, interpersonal and teaching skills.
- Demonstrates knowledge and understanding of quality monitoring, data collection, and performance improvement skills.
- Competent in use of Excel and other software.
- Current California driver's license and proof of auto insurance.
