Quality Improvement Coordinator

Reporting to the Clinical Integration Program Manager, the Quality Improvement (QI) Coordinator is responsible for overseeing the day-to-day implementation of the quality improvement and disease management programs and initiatives. Works closely with physicians, office staff, Clinical Integration (CI) medical leadership, payors and administrative data vendor to assist in facilitating meeting established quality improvement study submissions and targets.

Position Highlights:

  • Position: Quality Improvement Coordinator

  • Location: Warrenville, IL

  • Full Time/Part Time: Part Time (28 hours per week)

  • Hours: Monday-Friday, between 8am-4:30pm

  • Required Travel: Travel to Arlington Heights and other affiliate medical group practices across the EH System will be required

What you will do:

  • Serves as the primary contact with medical offices to support the successful completion of payor-defined QI and Clinical Integration program initiatives.

  • Distributes, presents, and explains specific study and protocol requirements and instructions to participating physicians and medical office staff.

  • Monitors various study participation activities, real-time data submissions, and communicates regularly with offices to reinforce the importance of initiatives and to assist them with meeting requirements.

  • Researches best practices, in conjunction with evaluating study-related office process flow, to recommend and/or develop tools or suggest changes that improve ease of compliance with participation requirements.

  • Follows up with offices as necessary to facilitate obtaining missing data and provides regular performance feedback on ongoing progress towards targets.

  • Completes data entry within the clinical integration tool and other systems as appropriate.

  • Works in collaboration with the CI care coordination team. Performs administrative tasks as needed, to help support the care management team as well as the CI team.

  • Serves as a liaison with all medical offices to coordinate the assessment and collection of appropriate QI data that meets CI or other study protocols and standards within the specified timeframes. Assists offices with submission of complete and comprehensive QI study data to payors.

  • Develops educational materials and provides in-service training to physicians and office staff in the areas of quality improvement. May also create disease management protocols with the CI team.

  • Assists in training medical office staff on the use of on-line quality improvement and clinical integration program management and tracking tools, including patient registries and portals, to help meet QI goals and performance targets.

What you will need:

  • Education: Bachelor's degree, required

  • Certification: n/a

  • Experience:

    • 1+ years of experience collecting, monitoring, analyzing, and presenting data in a meaningful manner

    • 3+ years of progressive responsible working knowledge of clinical quality improvement principles

    • 3+ years of experience working with EHRs, disease registries, and practice management software in an ambulatory setting is highly desirable

  • Unique or Preferred Skills:

    • Ability to work collaboratively with physicians, physician office staff, and payors

    • Strong communication skills and knowledge of a variety of presentations and training styles. Ability to tailor and present in-service training based on learning needs.

    • Ability to develop and write clear concise training materials and job aids

    • Proficiency with Microsoft Office including Word, Excel, and PowerPoint.

    • Ability to work effectively and independently in a team focused environment to achieve targeted goals

    • Familiarity and knowledge of principles related to primary care medical home and population management, preferred.
