We seek a Transitional Services Manager to provide effective service planning and transition processes for all youth while in the program and upon their return to their communities. Video: We Are AMIkids Website: www.AMIkids.org What you will be doing: Lead and supervise Multi-Disciplinary Team meetings to discuss each youth's needs, behavior, and progress with the established goals in the Performance Plan, ensure that all Department standards for case management are met, and all time frames are kept, Gather and analyze youth information from all program areas (mental health, education, medical, behavioral and skills development, etc.) to support in creation of individualized performance plans, transition plans and coordinate appropriate service delivery based on youth needs/goals, Schedule and oversee the transition process, including notification of involved parties, and formulating the Transition Plan, Maintain and oversee the youth Case Management file, ensure that all information is completed timely, and meets requirements for DJJ QI standards and CARF accreditation requirements, Deliver Evidence Based Delinquency Interventions and Restorative Justice Models adopted by the Program, adhere to fidelity requirements of evidence-based models per contract, Recruit, hire and train case management personnel; research and deliver instructional materials as needed; supervise and evaluate the performance and ongoing development of Case Management Staff, Foster and maintain working relationships with Community Resources, Stakeholders, Families and Youth to support service delivery at the program and coordination of services upon transition to the community, Role requirements: Bachelor's degree in psychology, Social Work, or other human service related field, Six consecutive months experience in a supervisory role required, Minimum 3 years' experience in a related field, preferably working with youth, Proficient in Microsoft Office (Excel, Word, Outlook, PowerPoint), Excellent verbal and writing skills.
