Family Experience: https://www.youtube.com/watch?v=hIGQYYVzDDQ Partners For Kids Website: partnersforkids.org The Care Coordination Nurse ensures quality patient care in the appropriate setting through care coordination, case management, coordination of hospice and home-based palliative care admissions, and transitions to different levels of care. The Care Coordination Nurse collaborates to organize individualized plans of care and communicates the plan to appropriate stakeholders. Practices professional nursing and promotes patient advocacy in accordance with the ANA Code of Ethics for Nurses. Maintains a current Ohio Nursing License. Why Nationwide Children's Hospital?
