The Family Support Provider is responsible for providing targeted support services, to provide a system of care that is child-centered with the needs of the child and family dictating the types and mix of services provided, to assist in keeping the family together and preventing an out-of-home placement. To be effective, the Family Support Provider works with children in their homes and communities, incorporating the entire family in the Wraparound and cultural heritage and utilization of the best practices model as communicated by the State of Oklahoma. The Family Support Provider maintains close communication with the Project Director, and other clinical and administrative staff in order to assure the highest quality of services is delivered to the juvenile consumer and his/her family. You will be working in an innovative and dynamic environment using a new transformative model of care Certified Community Behavioral Health Clinic (CCBHC). This model is characterized by innovative, team-based approach and whole person care for adults and children with a range of complex mental health and substance use challenges.
