Scope of Position

Reporting to the Director, Home-Based and Post-Acute Care, this position will be responsible for developing and implementing programs and services to further Ohio States home care and post-acute care strategies. This individual will play a key role in implementing existing Ohio State service offerings and also support established/emerging relationships within the home-based and post-care unit by serving as the key liaison between OSUWMC and external partners. The position will work with Administrators, Physician Leadership, Case Management Leaders, Nursing and others to successfully operationalize programs.

Position Summary

This position is responsible for supporting the Home-Based and Post-Acute Care Unit by creating and providing ongoing operational support to both internal programs and external partnerships. Working with colleagues from across OSUWMC, the Manager is responsible for providing planning, implementation and consultation services and will work to improve communication, key processes, policies, procedures and protocols. The position will be accountable for ensuring quality, patient experience, operational and financial performance targets are met across home-based and post-acute care businesses. This work requires extensive knowledge, skills and expertise in complex change and project management. This role is expected to be a collaborator, influencer and improvement advisor regarding home-based and post-acute care services.

Minimum Qualifications

  • Masters degree in health or business administration or related field is preferred, Bachelors degree is required.

  • Minimum of five years experience in a health system setting, preferably leading home care and/or post-acute care efforts.

  • RN or other clinical background preferred.

  • Knowledge of home and post-acute care network development practices, process improvement and cost management initiatives.

  • Effective interpersonal, verbal and written communication skills are essential.
