Description The Community Health Worker (and PD) provides overall support for the CARE Network clinical field staff in the delivery of case management services. Under the direction of the RN and Social Work team, provides case management support to program clients in the community/home setting, as well as assistance and support with office clerical functions as needed. CARE Network provides community based, comprehensive, disease management/case management services for the chronically ill, which may include: Heart Failure, Cancer, HIV/AIDS and other chronic diseases.
