Make a difference in the communities we serve as an ECM Lead Care Manager at our Venice Family Clinic. With guidance and direction from our Director of Enhanced Care Management (ECM), the ECM Lead Care Manager will provide high quality case management to eligible patients with complex medical and social needs. In this vital role, the individual will perform outreach and engagement, conduct a comprehensive assessment with each client, develop an individualized care plan for each client, and provide appropriate information regarding referrals and ensuring linkage.

Note, the incumbent may be required to work at our other Venice Family Clinic locations within Los Angeles County if needed.

Salary range: $30.53 - $40.30 Hourly

Qualifications

Required:



  • Bachelor's degree in social work from an approved program and 1 year of relevant experience OR equivalent combination of education and experience.
  • Knowledge of community resources, health education methods, advocacy, harm reduction and general clinical knowledge.
  • Ability to create and maintain boundaries with clients, and ability to discuss sensitive matters in a non-judgmental manner.
  • Ability to complete required program documentation, and follow through on assignments.
  • Ability to prioritize, and skill in managing multiple deadlines and demands.
  • Ability to work independently and as part of a group, and ability to work effectively with people of diverse cultural and educational backgrounds.
  • Ability to travel as needed; current CA driver's license, own transportation, and auto insurance.


Preferred: Oral and Written Bilingual language skills in English and Spanish
