General Purpose Under limited direction, the Community Outreach Specialist researches, develops, markets and coordinates community outreach and employee wellness programs. Monitors the ongoing progress for all programs, oversees associated budgets and reports on the desired outcomes. Represents the hospital and department on internal and external committees. The Community Outreach Specialist is accountable for assisting with the data gathering and timely completion of the Community Benefit Report (annually) and Community Health Needs Assessment (every 3 years) along with oversight for the UMH Health-West farmer's Market. Essential Duties
- Develops strategies for community health, outreach and employee wellness programs and services that include lectures, screenings, classes, etc. Provides oversight to community health and employee wellness events, speakers' bureaus and other initiatives. Works with Director to develop and monitor budget.
- Designs and delivers health education opportunities for community and employees. Continually tracks, measures and evaluates the effectiveness of all initiatives.
- Coordinates the annual health risk assessments for employees and manages all details involved.
- Oversees the data collection and review for the Community Impact reporting.
- Participates in internal and external teams overseeing community health, i.e.; Community Health Needs Assessment, Community Transformation Grant and other groups and organizations.
- Serves as community liaison for community health initiatives at the UMH-West Conference Center.
- Assists in researching developing and implementing programs for which grant funding is needed or received including grand writing and providing oversight for grants awarded.
- Identifies, elicits participation and assigns UMH-West staff, management and/or physicians to support community events.
- Oversees and coordinates the UMH-West Farmer's market.
- Develops and implements community or health system-based projects.
- Assists with recruiting and managing internal and external interns and volunteers as needed.
- Performs other duties as assigned. These may include but are not limited to: Maintaining a current knowledge base of department processes, protocols and procedures, pursuing self-directed learning and continuing education opportunities, and participating on committees, task forces, and work groups as determined by management
Job Requirements
General Requirements |
* Certified Health Education Specialist / Master Certified Health Education Specialist (CHES/MCHES) preferred |
Work Experience |
* Two (2) years of experience in community outreach, wellness, program administration or closely related experience required. * Experience overseeing wellness or community outreach initiatives, health screening, and / or program delivery in a healthcare setting highly preferred. * Training / certification in clinical health evaluations, including body composition, fitness testing, cholesterol, and blood pressure measurement. |
Education |
* Bachelor's degree in Public Health, Public Administration, Health Promotion, Health Science or other related field. |
Specialized Knowledge and Skills |
* Excellent communication skills (verbal and written). * Superior customer service and relationship skills with a commitment to excellence. * Demonstrated ability to be self-motivated and work with minimal supervision. * Strong organizational, coordination and follow through skills. * Demonstrated use of computers, data entry, Microsoft products and CBISA software. * Current CPR and First Aid certification preferred. * Ability to adapt to current and future trends in community wellness. * Management of the department budget. * Train and manage interns and future employees as needed |
