Case Manager - Rhode Island Works (Full Time) This position provides case management and care coordination services to individuals and families within the RI Works program, ensuring their compliance for specific component areas and assisting with resolving barriers for progress. We are looking for someone that has a caring nature, can demonstrate case management skills, emergency intervention methods, understand trauma, oppression and the impacts it has on an individual and family. Strong oral and writing skills needed and Bi-Lingual Spanish a huge plus! 1. Major Responsibility: Case Management. Supporting Actions:
- Conduct comprehensive assessments (including assessing risk such as but not limited to basic human needs, vocational/employment factors, mental health/substance use and other related risk) on assigned clients.
- Develop a service plan with clients in accordance to provisions stated within the specific case management program.
- Provide community and/or home visitation to clients on caseload.
- Provide instruction in adult daily living skills as needed and as related to barriers for academic/vocational/employment goals.
- Assist with completion of emergency funding, housing, utility assistance, medical and state benefits and other appropriate applications for clients as needed.
2. Major Responsibility: Work with stakeholders Supporting Actions:
- Collaborate with other CAP programs and administration to ensure coordination of efforts and resources and the smooth implementation of services.
- Maintain productivity levels, which are determined by Director and are in compliance with both program and CCAP and subcontract cap agency standards.
- Attend program-related meetings as appropriate.
- Attend and participate in all public relations activities as appropriate.
- Attend training and development sessions.
3. Major Responsibility: Serve as primary coordinator of various program databases. Supporting Actions:
- Maintain required client contact as deemed by program/DHS.
- Maintain current and accurate record keeping for client files in accordance with standards of particular case management program, Medicaid, and accreditation where applicable.
- Utilize computer database systems to maintain appropriate information on clients, per program and agency requirements.
- Complete all paperwork associated with maintaining current client statistics.
- Ensure agency standards of care are met and in compliance with funders and others as required.
- To complete all paperwork required for data generation in a timely fashion as determined by Director.
EDUCATION AND TRAINING REQUIRED:
- Bachelor's degree or minimum of 2 years related experience preferred.
- Demonstrated case management skills and experience; emergency intervention methods, family dynamics, health, mental health and substance abuse systems; and knowledge of community resources.
- May require in the future a case management/community health worker certificate must be willing to obtain certification if required.
- Bi-lingual highly preferred.
