Tidewell Hospice, a member of Empath Health is currently seeking a Psychosocial Team Leader (PSTL) to join our team.

The Psychosocial Team Leader (PSTL) is a qualified Social Worker who is responsible for coordinating and evaluating the psychosocial component of care for the Interdisciplinary Team. The PSTL is a preceptor to new staff on the team, providing education and mentoring. The PSTL provides ongoing clinical supervision of team cases and works directly with patients and families. The PSTL works to build a highly functioning, effective team, supporting change management and conflict resolution. The PSTL provides leadership and support in coordinating all team functions in support of the team leadership group including QAPI, team meeting facilitation and other leadership activities for the team.

POSITION QUALIFICATIONS/REQUIREMENTS:

  • Education and/or Experience: Masters Degree in Social Work from an accredited school of Social Work and three (3) years Medical Social Work experience or counseling related field.
  • Licensure:
    • LCSW
    • Valid Florida State Driver's License
  • Continuing Education: As required for licensure.
  • Prior Hospice experience preferred.
  • For field-based positions, employees must have reliable transportation which will enable them to perform tasks and responsibilities in a timely and appropriate fashion. Must provide proof of valid automobile insurance, a copy of which will be placed in the employee's HR file.

JOB DUTIES/RESPONSIBILITIES:

  1. Provides team leadership through the planning, organizing, directing and facilitating of the Interdisciplinary team (IDT), creating team assignments and insuring adequate after hours coverage. Ongoing collaboration performed with the CTM to determine and evaluate team goals and the psychosocial component of care.
  2. Collaborates with the IDT for the completion of the initial comprehensive assessment identifying the psychosocial needs and wishes of the patient/family, and assisting the patient/family to develop the plan of care. Provides an ongoing comprehensive assessment of physical, psychosocial, emotional and spiritual needs related to the terminal illness, the nature and condition causing the admission, factors affecting care planning, coping strategies, family dynamics and communication patterns, financial resources, family support systems, and the functional status of the patient to participate in their own care and caregiver's ability to function effectively.
  3. Supervision of psychosocial staff including clinical supervision, hiring of new staff, performance appraisals, and the dismissing of staff members according to performance standards agreed upon in collaboration with the CTM.
  4. Team coordination of bereavement services to ensure agency standards are at the highest level. Chart audits and monitoring of counseling services for the bereavement component of care. Develops the bereavement plan of care based on the family needs after the death and coordinates ongoing bereavement counseling as requested.
  5. Enhances the clinical practice of staff by acting as a preceptor, providing them with a consistent role model, mentor, instructor and resource person. Empowers psychosocial staff to apply their skills more effectively through mentoring and supervisory conferences.
  6. Maintains electronic clinical record and follows organization guidelines for completing documentation.
  7. Reviews and approves staff time sheets and documentation, auditing for completion, quality and regulatory compliance.
  8. Promotes clear communication between psychosocial staff and leadership or management. Responsible for the flow of information to and from team psychosocial staff.
  9. Attends Leadership meetings, PSTL meetings and Quality Management meetings, completes quality measures and initiates strategies to enhance quality of care in cooperation with the CTM.
  10. Promotes the psychosocial perspective and creates a positive culture in the IDT meetings and agency functions by being a role model for the psychosocial component of care, as well as, recognizing, reinforcing, and respecting the roles of all disciplines in the IDT.
  11. Supervises aspects of the psychosocial care such as team readmissions, transfers across teams and discharges from the Empath Health programs of care. Assumes responsibility along with the CTM, for admissions consents and elections being completed and coaches staff on financial classifications.
  12. Provides bereavement group facilitation for the community to assist with caregiving issues or grief and loss issues. Coordinates to assure there are groups every bereavement quarter to meet survivor's needs.
  13. Continual assessment of quality customer (internal and external) satisfaction; to collaboratively develop and implement service recovery or problem solving.
  14. Educates the community regarding hospice care philosophy and concept through a case by case consultation with persons in the community or educational in service or seminar.
  15. Serves as a resource in the care community and provides outreach activities.
  16. Assume other responsibilities as requested by supervisor.

  18. To assure service excellence after hours, provides after hour care including on call rotation and assuring that the staff participate in on call rotations.
  19. Assumes other responsibilities as requested by the CTM or Director of Social Work Practices.
