The final salary and offer components are subject to additional approvals based on Cook County Health (CCH) policy. Your placement within the salary range is dependent on a number of factors including your work experience and internal equity within this position classification at CCH. For positions that are represented by a labor union, placement within the salary range will be guided by the rules in the collective bargaining agreement.

Job Title: Project Coordinator - Core Center

Dept: CORE Patient Services

Shift: 8:30AM-4:30PM

Job Summary NON-UNION

The Project Coordinator works with the CORE human immunodeficiency virus (HIV) and Hepatitis Clinic, outreach, and Cermak Health Services staff to screen and link patients to HIV/Hepatitis C Virus (HCV) care. The Project Coordinator is responsible for day-to-day operations to monitor progress towards meeting program goals, developing program materials, identify and resolve problems and implement solutions as necessary. The Project Coordinator works with the clinic multi-disciplinary team to link, engage and retain patients in HIV primary care.

General Administrative Responsibilities

Collective Bargaining




  • Review applicable Collective Bargaining Agreements and consult with Labor Relations to generate management proposals



  • Participate in collective bargaining negotiations, caucus discussions and working meetings




Discipline




  • Document, recommend and effectuate discipline at all levels



  • Work closely with labor relations and/or labor counsel to effectuate and enforce applicable Collective Bargaining Agreements



  • Initiate, authorize and complete disciplinary action pursuant to CCH system rules, policies, procedures and provision of applicable collective bargaining agreements




Supervision




  • Direct and effectuate CCH management policies and practices



  • Access and proficiently navigate CCH records system to obtain and review information necessary to execute provisions of applicable collective bargaining agreements




Management




  • Contribute to the management of CCH staff and CCH' systemic development and success



  • Discuss and develop CCH system policies and procedures



  • Consistently use independent judgment to identify operational staffing issues and needs and perform the following functions as necessary; hire, transfer, suspend, layoff, recall, promote, discharge, assign, direct or discipline employees pursuant to applicable Collective Bargaining Agreements



  • Work with Labor Relations to discern past practice when necessary





Typical Duties



  • Collaborates with leadership to support program development and implementation. Explain the primary objectives to key stakeholders. Serves as a liaison for updates on program outcomes.
  • Supervises and oversees staff to ensure program goals are monitored.
  • Communicates with the multi-disciplinary team members daily and serves as a contact person for patient's needs before the clinic visit, during and in between clinic visits.
  • Identifies and communicates to the team patients lost to care and those considered at risk for being lost to care.
  • Addresses operational issues with staff which may impact the clinic functions such as team members absences, who will be providing the services needed and how the flow of the clinic might be impacted.
  • Provide oversight in collaboration with Clinical Care Coordination Committee of clinical data for outcome measures and provides feedback to the team using a PDSA cycle to implement the changes as identified.
  • Enters and maintains submits reports specific to database related activities such as ensuring treatment information is accurate and current; HAT; positive HIV/HCV cases.
  • Prepares and submits quality data reports on program progress, continuous quality improvement measures, and linkage rates, when required. Presents to appropriate stakeholders, funder, and staff to provide updates.
  • Monitors and ensures quality measures are taken and takes corrective as needed to address errors. Participates in continuous quality improvement efforts.
  • Assists with making reminder calls to patients regarding appointments, labs, and medication adherence issues, when needed.
  • Develops and implements the hepatitis hotline and tracks outcomes.
  • Works with the Hepatitis Clinic staff as a team member in providing patient centered HCV screening and linkage to care.
  • Works with CCH/HIS to develop emergency medical record (EMR) prompts and educational Cerner materials. Develops EMR prompts and educational materials related to HIV/HCV. Assists with manuscript development and program marketing.
  • Works closely with Cermak administration to implement lab procedures and follow up of positive HIV/HCV positive results.
  • Provides patient, provider and community education on HIV/HCV and related topics to help implement routine HIV opt-out testing throughout CCH.
  • Encourages HCV testing in the patients who fall within the testing birth cohort of 1945-1965 throughout CCH and provide guidance with linkage to care at the CORE Center HCV Assessment Clinic.
  • Develops and helps implement HIV/HCV policies and procedures.
  • Provides training to clinical staff to help engage them in routine HIV opt-out testing and birth cohort HCV testing.
  • Prepares and submits linkage data into system-wide database on positive HIV/HCV cases within CCH and perform routine quality assurance on data provided.
  • Enters linkage data into database, to include but not limited to patient viral load, cluster of differentiation 4 (CD4) counts, risk factor and outcomes.
  • Performs Quality assurance on previously entered data to ensure data is most current and accurate for newly and known cases of HIV/HCV.
  • Attends and presents at meetings, presentation and committees when necessary.
  • Performs other duties as assigned.

Minimum Qualifications



  • Do you possess a Bachelor's degree from an accredited college or university? (Must Provide transcripts at the time of interview)
  • Do you possess two years or more of experience in health care setting providing health education in the HIV field? Required
  • Do you possess Project management experience? Required
  • Position requires moderate travel for which the employee must have a valid driver's license in good standing and access to an insured vehicle. Required


Preferred Qualifications



  • Do you possess a Master's degree from an accredited college or university? Preferred
  • Do you have prior experience in the hepatitis field, emergent care settings, and familiarity with justice involved populations such as detainees or clinical research? Preferred
  • Do you have familiarity with infographics, Microsoft Office (Power Point and Excel)? Preferred
  • Are you Bilingual? Preferred

  • Knowledge and understanding of principles of health communication, including social marketing and determinants of community health and how to build local capacity



  • Excellent verbal and written communication skills necessary to communicate with all levels of staff and a patient population composed of diverse cultures and age groups
