Since 2001, Spring Mountain has provided acute crisis stabilization and outpatient services to the Las Vegas community and beyond. We offer a caring and healthy environment, with 24-hour care that allows individuals to make positive changes in their lives. In addition to crisis stabilization for patients as young as age 5, we offer treatment and detox for co-occurring substance abuse issues in teens and adults. Spring Mountain Behavioral Services has two locations to serve the diverse community of Las Vegas, Nevada. Spring Mountain Treatment Center is currently seeking a dynamic professional to join our team as a PRN Licensed Case Manager. The Case Manager provides quality case management and psychotherapeutic services to Spring Mountain's populations. They serve as a member of multi-disciplinary team supporting the hospital's treatment program and philosophy and assure the deliverance of quality treatment to patients and their family members. The Case Manager provides social histories, discharge planning, case management and psycho-educational/process groups to ages specific populations.

JOB DUTIES/RESPONSIBILITES:

  • Conducts psychosocial assessments to document history and identify preliminary issues for treatment focus by interviewing the patient and their family members.
  • Coordinates care of assigned patients throughout their stay at SM and evaluate individualized safe and timely patient discharge needs.
  • Coordinates referrals for these services and utilizes interpersonal skills to maximize excellence in customer service, safe patient care, and professionalism.
  • Develops and coordinates an individualized discharge plan for the patient by utilizing team and chart information to determine the patient's aftercare needs.
  • Works with Discharge Planner to develop, implement and document a discharge plan established with the patient per TJC standards and with attention being focused on progressing toward discharge.
  • Attend Treatment Team Meetings as determined by the Director of Clinical Services
  • Provides group process-oriented therapies as well as conducts educational and other didactic groups for patients using various professional treatment modalities.

Qualifications

REQUIRED KNOWLEDGE, SKILLS AND ABILITIES:

  • Master's degree in social work
  • Current licensure in the State of Nevada; also acceptable licensure: CSW-I, LCSW
  • Experience with age-specific inpatient setting preferred
  • Experience in taking psychosocial history and case management
  • Knowledge of developmental psychology of adolescent and adults and have the capacity to identify pertinent clinical information
  • Possess knowledge of clinical issues related to mental health patients and their discharge planning
  • New hires are required to attend 1 full week of New Employee Orientation on day shift, normally 8:00 am - 4:30 pm Monday through Friday
  • Must successfully pass background check (includes criminal history, Child Abuse and Neglect Registry, and ABI/FBI fingerprint check) and drug screen
