Summary

(Completion of an ASHP accredited PGY2 Pharmacy Residency in Pharmacotherapy, Cardiology or Critical Care is preferred).

Under the general direction of the Director of Pharmacy Clinical Services, or theirdesignee, the Clinical Pharmacist reviews drug therapies, formulates recommendations based on current literature andestablishedstandards of practice, directly supervises supportive personnel, serves as a resource for drug information; participates and/or directly supervises the preparation and distribution of medications, and serves as a member of the healthcare team with theprescriberand direct caregiver. This position will be primarily responsible for the provision of comprehensive pharmacy services for adult and pediatric patients in a variety of critical care units, with some support for other patient populations and pharmacy operations as needed. Coverage includes medical intensive care, surgical intensive care,neurointensivecare, cardiac intensive care, cardiothoracic intensive care and pediatric intensive care units.

This position requires a high degree of interaction with the interdisciplinary team. The clinical pharmacist should be prepared to discuss complex clinical cases with physicians,advancepractice providers, nurses, and other relevant staff involved to assist in the development of care plans for the patient. You will also be responsible for responding to adult medical codes, including adult code strokes, where you may be asked to simultaneously prepare drug therapies and field drug information.

The pharmacist in this position will also be responsible for supporting and facilitating quality and safety initiatives, training pharmacy learners; developing policies, procedures and guidelines usingevidence basedmedicine and national quality indicators; and engaging inevidence basedresearch.

This position is a rotatingdaysand night coverage. The night shift will include a 7-day on-7 day off rotation during a specified block.

Required Minimum Knowledge, Skills, and Abilities (KSAs)

1. Education: The Clinical Pharmacist role reviews medication orders for safe and effective medication use; collaborates with physicians and other health care personnel to achieve optimal patient outcomes; and formulates recommendations based on current literature and evidence-based practice.

2. License/Certifications: Current applicable state(s) license with Board of Pharmacy required.

3. Experience: Previous pharmacist experience preferred.
