This position is a hybrid position with majority of work done remotely. Some travel to VelocityCare locations may be required.

The RN Nurse Navigator promotes Carilion's culture of service excellence and quality by implementing and accepting accountability of the service standards. The RN Nurse Navigator performs daily review of outstanding labs and radiology reports, and reports discrepancies to the VC physician assigned to URI follow up. The RN Nurse Navigator contacts patients to instruct on appropriate follow up. Other duties include, but are not limited to the following:

  1. Performs daily reviews of outstanding labs and radiology reports, and reports discrepancies to the VC physician assigned daily to URI follow up as outlined in follow up protocols. Performs telephone contact with patients to instruct on follow up or changes in plan of care.
  2. Contacts patients to instruct them on the appropriate follow up. Attempts daily contact with patients and provide service recovery by assisting with arranging appropriate follow up care and complaint resolution.
  3. Maintains a thorough knowledge of the Velocitycare/Ambulatory clinical information systems for documentation of the content of telephone encounters and generating daily reports
  4. Demonstrates the ability to work independently with minimal direction, prioritize and meet deadlines. Brings questions to manager.
  5. Provides age appropriate education on an as-needed basis to patients during contact conversation.. Education is specific to patient need, and will be well documented.
  6. Education is incorporated into the plan of care and includes physicians, staff, population specific representatives about patient care options.
  7. Provides a high level of customer service to patients, visitors, clinical staff and colleagues. Builds strong relationships and team cohesiveness, focuses on quality and positive solutions, communicates respectfully, demonstrates compassion and understanding in response to patient/customer needs. Adheres to customer service standards, and patient confidentiality policies.

What We Require:

Education: Graduate of accredited school of nursing required.

Experience: Minimum of 3-5 years experience in Nursing required; 5 years experience preferred.

Licensure/Certifications/Registration: Current licensure as a Registered Nurse in the State of Virginia required. AHA BLS-HCP required.

Other Minimum Qualifications: Proven ability to understand medical terminology, problem solving and handle stressful situation with a professional manner. Good team dynamic skills in order to facilitate change and achievement of defined outcomes. Must be able to work with minimal amount of supervision. Demonstrates a positive demeanor, excellent written and oral communication skills, and a professional appearance and approach. Excellent organizational ability, and initiative. Must have the ability to problem-solve and communicate with a wide variety of people in an effective manner. Able to work independently with minimal direction, prioritize and meet timelines. Must exhibit skills to utilize automated computer systems, including electronic medical records. Keyboarding skills required for automated computer systems, including electronic medical records
