Scope of Position

Responsible for verifying new medication orders, checking prepared medications, overseeing dispensing from the pharmacy, and completing clinical patient review for safety and accuracy. Provides safe, efficient, high quality, patient-centered, and evidence-based medication services to all patients. In cooperation with other pharmacists and pharmacy technicians, acts as a resource and provides answers to drug information questions for patients and family members, pharmacists, nurses, physicians, advanced practice providers, medical students, undergraduate and graduate pharmacy students, and other associated hospital personnel. Screens for drug-drug, drug-herb/dietary supplement, drug-allergy, drug-food, drug-disease state, and drug-laboratory interactions. Obtains and maintains clinical privileges, if applicable. Using clinical privileges, adjusts medications based on route of medication administration access, monitors and adjusts medications based on renal, hepatic, antithrombotic indications, and hematologic parameters, orders and adjusts laboratory tests related to monitoring medication therapy, and modifies and adjusts Total Parenteral Nutrition therapy and associated fluids and electrolytes in conjunction with a dietician, if applicable to role. Participates and provides support in the Emergency Response effort, including Code Blues and Stroke Alerts. Engages in appropriate hospital and/or departmental committees relevant to practice area.

Position Summary

This position functions to coordinate all aspects of patients' pharmaceutical needs at The Ohio State University Wexner Medical Center. Duties include but are not limited to medication order approval and distribution; assurance of appropriate dose, route, frequency, and duration of therapy; clinical profile review to assess safety and accuracy of medication services; patient medication profile review for drug-drug interactions, duplication of therapy, drug allergies and appropriate regimens; coordination of patients' pharmaceutical needs with physicians, advanced practice providers, and nurses; utilization of clinical pharmacy privileges to promote optimal medication practices; contribution to Emergency Response efforts alongside the healthcare team. Must be able to demonstrate knowledge and skills necessary to provide care appropriate to the age of the patients served. This position is also responsible for overseeing technicians, undergraduate and graduate student teaching, and providing educational programs for hospital staff.

Minimum Qualifications

Bachelor's Degree in Pharmacy. Registered Pharmacist. 0 years of relevant experience required. 0-2 years of relevant experience preferred.
