Responsibilities
Coordinate and provide services and resources to an pregnant individuals/families necessary to develop and facilitate a personalized intervention plan based upon barriers and health care issues identified through telephonic and/or on-site screening process. Adheres to policies, procedures, and regulations to ensure compliance and patient safety.
Qualifications
Job Requirements:
- High School Diploma
- Medical terminology
- Knowledge of community and state resources
- One (1) -Three (3) years of recent related experience
Preferred Job Requirements:
- Associate degree or Degree from an Accredited College
- Bilingual-English/Spanish preferred
