Sr Lic Vocational Nurse Lubbock 39243BR Position Description Employees in this classification are experienced LVN's responsible for performing a wide variety of patient care activities in an assigned specialty or subspecialty outpatient clinic. Work is performed under the direction of a staff nurse and/or physician as part of a total health care team in providing complete evaluations to patients. Works under the direct supervision of staff nurse or physician. No delegated supervisory authority; may instruct co-workers and supervise them in the performance of special procedures. Work is performed under usual clinic conditions, necessitating a high degree of contact with patients, families and clinic personnel. Major/Essential Functions Reports data, identifies and reports alterations in patient responses to assist in the identification of problems and formulation of goals/outcomes and patient-centered plans of care in collaboration with patients, their families, and the interdisciplinary health care team. Patient clinical information/critical test results are conveyed in an accurate and timely manner. Communicates patient data using EMR to support decision-making to improve patient care. Appropriately documents care provided and patient's response to care in an appropriate and timely manner. Implements teaching plans for patients and their families with well-defined health learning needs. Patient teaching is appropriate to patient's communication and literacy needs, and setting and documented in EMR. Takes and records accurate vital signs and measurements. Vital signs and measurements are accurate and consistent with type of exam or procedure conducted. Communicates with patients via telephone per clinic protocols. Telephone communication includes the following: Providing answers to questions concerning health problems, disease, and treatment in a caring and informative manner. Following ambulatory clinic policy regarding documentation of communications with providers, staff, and patients. Accurately and proficiently screening incoming patient telephone calls for urgency of situation and what action should be taken. Assists in determining the physical and mental health status, needs, and preferences of culturally, ethnically, and socially diverse patients and their families. Assessment is performed within scope of practice and as defined by organizational and departmental guidelines. Prepares and operates necessary equipment and instruments for examination, treatments, and procedures. Equipment and instruments are used and maintained in accordance with established procedures in clinical specialty area. Attends nursing staff meetings. Attends departmental nursing staff meetings to review issues, processes, communicate new and pertinent developments, address performance improvement initiatives. Sterilizes and disinfects medical equipment and instruments. Sterilization or high level disinfection of instruments is conducted in accordance with ambulatory clinic policies and manufacturer's recommend. Maintains inventory of supplies in examination and procedure rooms. Ensures supplies in examination and procedure rooms are not expired. Completes Annual Nursing Core Competency Training and Skills Fair. Participatory document is present in Employee's personnel file. Participates in emergency preparedness drills in department. Responds and participates appropriately to the situation and according to HSC policy. Assists with or performs exams and procedures and documents intervention in EMR. Exams and procedures are conducted within scope of practice, training, and departmental policies; EMR reflects accurate notation. Specimens taken during exam or procedure are labeled at patient's bedside. Participates in the identification of patient needs for referral to resources and facilitate continuity of care. Continuity of care issues/barriers/concerns are addressed and documented in EMR in an accurate and timely manner. Meets the minimum requirements for professional development as outlined by the SO< Education Plan. 2. Ensure 100% compliance of full requirement of the SOM Educational Plan for yourself. 3. Evaluate your progress by reviewing quarterly the ACME Transcripts of Training as well as any training certificates from other sources verifying attendance. Required Qualifications + Education:+ Completion of a Board of Nursing accredited program of vocational nursing. + License/Certification:+ Current Vocational Nursing license in the state of Texas or the attainment of a temporary permit from the Board of Nursing to practice vocational nursing pending full licensure in the state of Texas. + Experience:+ At least two years of LVN experience in a hospital, clinic or community-based environment.
