Case Management Educator Job Responsibilities:
- Plans and implements case management focused educational activities to support on-boarding, orientation, individual development and organizational goals. Utilizes system strategies and tools to assess competence of new case management team members while focusing on quality of care and patient safety goals.
- Implements and participates in Inova Health System programs for case management team members to achieve patient care and safe discharge outcome standards,
- Creates educational experiences for both new and experienced team members through integration of theoretical knowledge including adult learning theories, evidence-based principles, and clinical expertise.
- Informs appropriate Case Management leaders when team members need support, are unable to successfully complete, or are non-compliant with required competencies.
- Works with teams to synthesize data about quality and safety measures which lead to needed improvements in case management standards of care by developing, revising and implementing polices, procedures and protocols.
- Collaborates with Director, Managers and Physician Advisors to ensure practice is evidence-based and identifies, reviews and applies evidence to guidelines and standards for patient care.
- Performs other duties as assigned.
Additional Requirements:
- Education: Bachelor of Science in Nursing (BSN) or Masters Degree in Social Work (MSW)
- Experience: A minimum of five years Case Management experience
- Certifications: Accredited Case Manager or Certified Case Manager within twelve months of hire
- Licensure: Licensed in the Commonwealth of Virginia as a Nurse or Licensed Clinical Social Worker
- Additional Requirements: Basic Life Support through the American Heart Association upon start
