Role: Weekend Peer Case Manager

Reports To: Director
Schedule: Friday-Tuesday, 8:00am-4:30pm

Summary

With the support and in partnership with the E6 team, the E6 Peer Case Manager will support the E6 team successfully engage with people currently living on the streets, for the purpose of getting them matched to permanent, affordable housing. Peer Case Managers will walk the streets of Metro LA (Service Area 4) in teams to identify, engage and assist in housing homeless residents. These teams are part of a new large-scale street-based engagement effort in the Metro Los Angeles Area (Service Area 4). This is a LA County approved strategy to combat homelessness. The goal of E6 is to create a countywide network of multidisciplinary, integrated street-based teams to identify, engage, and connect, or re-connect homeless individuals to interim and/or permanent housing and supportive services.

Essential Duties & Responsibilities

1. Conducting outreach to people experiencing homelessness living in Metro LA (Service Area 4)

2. Assessing the housing needs of people living on the street

3. Assisting clients in the process of getting affordable housing

4. Advocating on behalf of client's needs

Document and account for time spent on client contacts and activities in client record in accordance with agency and funder standards

6. Attending team meetings and program-wide meetings daily

7. Providing support to nurses and social workers on the team with their client needs

8. Accompanying clients to appointments

9. Providing resource and referral information to clients

Maintain appropriate boundaries at all times

Qualifications

1. An eagerness to provide top-notch quality services to individuals experiencing homelessness.

2. Lived experience with homelessness, incarceration, or substance use.

3. An understanding of and familiarity with homeless services and affordable housing in LA county

4. Experience in the fields of social work, public health or other types of outreach

5. Training in harm reduction, "housing first" principles and street-based outreach preferred

High School diploma or GED preferred

7. Peer Counselor Training preferred



  1. Valid CA Driver's license, reliable car, auto insurance, and an acceptable driving record preferred
