Position Description:

In the role of Veteran Outreach Wellness Coach you will be a team member of the Captain John D. Mason Veteran Peer Outreach program; coordinate activities with multi-disciplinary team involved in managing outreach to patients and coordination of patient care including with physicians, psychologists, peer specialists, social workers and emergency/inpatient services.
Primary Functions
- Monitor patient care from initial contact in the community through referral/coordination with outpatient, inpatient and/or emergency medical/psychiatric services to ensure patient progress towards desired outcomes.
- Collaborate daily with peer specialists in the community to maximize quality care.
- Assist peer specialists with patient assessments.
- Communicate plan of care to patients including responding to emails, media posts, telephone messages and facilitating referral to peer specialists, outpatient care and/or emergency/inpatient services.
- Coordinate patient ongoing contact to support follow-up and referral to clinical services.
- Complete program evaluation summaries.
- Facilitate staff meetings and advisory committee meetings.
- Assist with the fiscal management of annual budgets.
- Provide outreach/education services to other professionals as appropriate.

- Knowledge of biology, clinical judgment, medicine, cultural awareness, and consulting.
- Medical documentation, interpersonal, practice-based learning and improvement, empathy, and decision making skills.
Specifications
Appropriate experience may be substituted on equivalent basis
Minimum Qualifications
- Bachelor's degree
- 3 years experience
- Certification: Wisconsin provider status or certification
Preferred Qualifications:
- Minimum 3,000 hours post-Bachelor's supervised experience
- Military Veteran
- Experience working with veterans
