Job Summary
The Population Health Specialist is responsible for outreach to patients with gaps in chronic disease management and preventive care. If you like owning your own schedule, like your work to vary from week to week, and a self-starter that can run with changes easily, this is the position for you. You must be able to meet patients where they are: clinics, home, work, or the parking lot of a restaurant. Will be required to travel across various counties to administer this eye exam: St. Clair Shores, Redford, Livonia, Westland, and Outfield counties. Mileage reimbursed for all travel. May be traveling up to 3 days per week. Essential Functions
