EDUCATIONAL/TRAINING REQUIREMENTS: CERTIFICATES, LICENSES, AND REGISTRATION: EXPERIENCE REQUIREMENTS: PRIMARY FUNCTION: ESSENTIAL FUNCTIONS: Collaborates and continuously partners with chronically ill or high risk patients and their family/caregiver(s), clinic/hospital/specialty providers and staff, and community resources in a team approach. Evaluates the needs of cancer patients and educate them about their cancer diagnosis Prepares patients and caregivers for what to expect throughout the cancer care continuum Creates and promotes adherence to a care plan, developed in coordination with the patient, PCP, oncologist and family/caregiver(s) Provides clinical support to Patient Navigators and Sidney Kimmel Cancer Center (SKCC) patients Interacts with co-workers, visitors, and other staff consistent with the values of Jefferson. OTHER FUNCTIONS AND COMPETENCIES: Educate patients and families about disease process and potential side effects Assist patient with treatment decision making; develop and use decision aids as appropriate Attend and support Tumor Boards Reduce emergency room utilization and hospital readmissions through outpatient symptom management Follow patients throughout the care continuum including inpatient admissions in collaboration with inpatient care management resources Support transitions of care and coordination between SKCC sites Provide clinical consultation to the Patient Navigators Educate multidisciplinary colleagues about the role of the Nurseand Patient Navigators, identified barriers, available resources and the navigation process Support program growth and patient experience in all patient interactions Covid Vaccination is a requirement for employment at Jefferson for employees working at Jeffersonrsquo;s clinical entities or at the University.
