The Digestive Health Nurse Coordinator is a Registered Nurse, reporting to Nursing leadership within the Digestive Health Center and under the direction and supervision of Nursing leadership. The primary role of the nurse coordinator is to ensure seamless coordination of patient care for Digestive Health Care patients. This includes clinical practice, care coordination, education, quality improvement, with significant latitude for exercising professional judgment. EEO/AA/Disability/Veteran Responsibilities
Qualifications EDUCATION Registered Nurse in State of Connecticut. Bachelor's Degree in Nursing preferred, EXPERIENCE At least 2-3 years experience in a gastroenterology inpatient or outpatient setting, including experience in coordinating services and care for gastroenterology disorder patients. LICENSURE Registered Nurse in State of Connecticut. SPECIAL SKILLS Excellent interpersonal and organizational skills. Ability to assert oneself, advocate for patients and their families, work independently with minimal supervision. Ability to manage multiple priorities. Computer experience and knowledge of word processing and data entry systems. PHYSICAL DEMAND Must be flexible to meet the demands of the specialty, outpatient clinic session, specialty and patient/family needs. YNHHS Requisition ID 133531 |
