The Licensed Practical Nurse I provide care to patients and is responsible for performing designated tasks and responsibilities under the direction of a Registered Nurse and with a focus on supportive and restorative care, health counseling and teaching. The Licensed Practical Nurse I is knowledgeable about and supportive of Patient and Family Centered Care Model using a collaborative approach with the Register Nurse. The LPN I demonstrates clinical judgment by applying problem-solving techniques and by integrating skills and knowledge into clinical practice. Patient and family centered care (PFCC) at YNHH is demonstrated by working with patients and their families based on the 4 principles of PFCC: participation, dignity and respect, information sharing, and collaboration. This includes providing service excellence by creating a great "First Impression" by demonstrating exemplary customer service skills for all customer groups including patients, families/friends, physicians, staff and support department personnel. Responsibilities
- 1. Provides direct patient care with the establish model of nursing as evidenced by the following:
- 1.1 Documents admission information under supervision of the Register Nurse
- 2. Collaborates with the Register Nurse and other members of Health Care Team.
- 2.1 Collaborates consistently with Register Nurse to develop the patient-s plan of care and implement interventions on all assigned patients.
- 3. Verifies, prepares and administers medication to all assigned patients to ensure safe medication delivery and positive outcomes.
- 3.1 Clarifies any questions regarding physician prescriptions orders immediately to the Register Nurse 100% of the time.
- 4. Participates in team building activities that contribute to the patient care, staff education, and self development.
- 4.1 Participates in interdisciplinary conferences
- 5. Maintains clinical competency to ensure that practice is evidence- based.
- 5.1 Attends all mandatory and continuing educational classes.
Qualifications
EDUCATION Graduated from an accredited LPN program. Current license in the State of Connecticut. EXPERIENCE Prefer experience in a clinical procedural area. Must complete an approved medication course and forty (40) hours of supervised medication administration including medication verification, transcription of physician's prescriptions, preparation and administration of medications in accordance with department policy within one (1) year of employment with Y-NHH. Must also demonstrate competency in designated hospital and unit PHYSICAL DEMAND Requires prolonged standing; walking; use of sight, hearing and touch; and moving/lifting patients of varying weights; CPR.
Additional Information
Previous ambulatory clinic LPN experience preferred not required.
