Location:Orion Building -6630 Orion DriveFort Myers FL 33912 Department: Administration Services - CCT Work Type: Full Time Shift: Shift 1/8:00:00 AM to 4:00:00 PM Minimum to Midpoint Pay Rate:$80,038.40 - 104,020.80 / Annual SummaryThis position is responsible for providing supervision to the Social Work and Community Health Coordinator team(s), within the Center for Care Transformation Nurse Navigation team, as they act in a connector role between the patient, the healthcare system and the community. This role will collaborate with all teams in the Center for Care Transformation and externally in the community. Oversight includes validating team members are performing appropriate outreach, evaluation of social determinants of health, planning, organizing, implementing, and follow-up of activities which support enhanced patient centered care; via coordination of community-based care with multiple post-acute care settings (i.e. home, hospital, office and SNF/SNU locations). This role will support value-based care initiatives, including, but not limited to right care, right cost, right place and right time. While performing all aspects of their job in their related field, the Supervisor, Nurse Navigation social work and community health coordinator team, will exhibit critical thinking and problem solving. He/she will serve as a clinician, teacher, researcher, team leader and professional development coordinator (i.e. responsible for the scheduling, supervision, training and development of the Social Work and Community Health Coordinator teams to facilitate smooth operational functioning). This position will have a hybrid schedule. RequirementsEducation: Bachelors degree in social work or nursing required. Masters Preferred. Experience: Five (5) years of Social Work or Nursing experience in medical or healthcare setting preferred. Certification: Certification in appropriate specialty preferred. License: Must possess any certifications/licensures required by State regulations. Other: Knowledge of care planning, Medicare/Medicaid regulations, social determinants of health, and exhibit good communication and organization skills.
