Description

Woodbury Health Care Center, a Lifespark Skilled Nursing Community, is looking for a dedicated and passionate TCU Social Worker to join our growing team.

Woodbury Health Care Center is home to seniors requiring TCU (Transitional Care Unit) Care and Long-Term Care. We are in Woodbury, right next door to Woodbury High School. We have been helping seniors Age Magnificently since 1979 and are part of Senior Care Communities, a local non-profit supporting seniors.

Shifts Available: Full Time, days

Wage: $25-$28

In this position you will be responsible for the operations of the Social Services department, providing for each resident's social, emotional, and psychological needs and the continuing development of the resident's full potential from admission to discharge. Duties include:



  • Develop a social history, social assessment and care plan which identifies pertinent problems and needs, realistic goals to be accomplished, and the specific action to be taken in resolution of the problems and/or needs upon admission of each new resident.
  • Participate in the pre-admission screening of all potential residents and provide information to residents and families during admission.
  • Orient the resident to the long-term care environment and facilitate adjustment upon placement.
  • Document progress notes which relate to each resident's care plan when necessary.
  • Assist the resident and resident's family in discharge and placement planning.
  • Organize family groups to promote communications, education and support between family members, facility staff, and administration and provide counseling, as needed.


Preferred Qualifications:



  • Post Secondary Education with a Bachelor's degree in a related field.
  • A minimum of two years in long-term care or acute care is preferred.
  • Must be able to relate professionally, positively, and cooperatively with residents, resident's families, and employees.
  • Must possess advanced computer skills.
