Primary duties:
* Coordinate and manage morning postpartum sign out with resident/attending physicians and CNMs * Perform postpartum patient rounding and management of postpartum patients including, but not limited to the following functions: a.Physical assessments b.Daily progress notes c.Ordering and following up indicated diagnostic tests d.Contraceptive counseling and management, including placing Nexplanons and arranging outpatient contraception e.Placing inpatient consults and discussing patient with consulting services f.Working with Breastfeeding Medicine to support infant feeding g.Working with inpatient Social Worker as needed in obtaining patient resources and discharge planning h.Working with pediatric teams to coordinate mother-baby care * Manage postpartum pager when on shift: respond to pages, assess patients, place orders and update LIP teams as necessary * Perform discharge education, discharge planning and write discharge summaries * Facilitate communication between LIPs and Mother-Baby nursing and staff * Work directly with students assigned to the unit as required and participate in teaching. * Opportunity to perform circumcisions in rotation with OB resident teams and pediatrics services. Conduct post-partum tele visits per ACOG guidelines. Coverage of outpatient postpartum clinic if needed to support Fourth Trimester goals. Secondary duties: * Serve on select committees as needed based on professional interests. * Participate in the clinical experience of students (graduate/post graduate advanced practice, nursing, and medical students), interns, and residents, as appropriate. * Participate in group, peer and other review activities as requested. * Prepare and attend to all reports, claims, and correspondence necessary or appropriate to the performance of professional services. * Participate in research projects and quality improvement initiatives that impact patients on the Mother/Baby unit. Participation includes understanding of active projects and if necessary, supporting clinical research coordinators assigned to recruit patients. Summary of Expectations: * Work in conjunction with the medical director, administrative management, nursing and other interprofessional team members to facilitate patient care and management throughout the health care system. * Exercise independent judgment in the assessment, diagnosis and initiation of medical processes and procedures commensurate with training and experience, as credentialed by UVA Health System. * Perform and document comprehensive health histories and physical assessments, to include initiation of CPR or other appropriate emergency therapy as needed. * Formulate diagnoses, identifies problems and health risk profiles, document and clearly communicate with other members of the health care team. * Perform, order, and interpret diagnostic studies, such as x-rays and laboratory tests, in consultation with peers and colleagues. * Perform procedures within scope of practice and UVA Health System credentialing and privileges. * Order medications, treatments and therapies in accordance with Virginia Prescriptive regulations. * Request consultation and initiate referrals to other health care providers, including specialists when and if needed. * Evaluate and analyze patient responses to the disease process and/or therapeutic, diagnostic interventions. * Provide information, education and support to patients/families regarding disease processes and treatment plans (diagnostic procedures, medications and treatments), thus, facilitating communication between patients/families and the health care team. * Manage patients in accordance with established UVA Medical Center and UVA UPG policies and procedures. * Report births, deaths, outbreaks of communicable diseases and other information critical to the patient's and/or the public's health as required by legal or governmental authorities, in a timely manner. * Meet quality standards as determined by the Medical Director and/or UVA Health System. * Promote the professional practice of the advanced practice group by serving as a clinical resource to nursing, medical and allied healthcare practitioners. * Work closely with learners of all levels (nursing, medical student, Resident, fellow, APP) in support of UVA Health's commitment to academic excellence. Schedule: This is a full-time position; hours will generally mirror pager turnover times of 7:00 AM in order to attend rounding and huddles. This is a 24/7/365 unit and due to the census and acuity, we expect the Inpatient APP staffing to support as needed, including weekend and holidays. Clinical:
* Independently and in collaboration with physician(s) and interdisciplinary team members, provides comprehensive clinical and organizational direction and leadership to the healthcare provider team in the care of patients. * Performs comprehensive history and physical assessment * Documents findings noting pertinent normal and abnormal findings * Orders and interprets the appropriate diagnostic labs and tests * Forms medical diagnoses based on history, assessment and diagnostic findings * Develops, implements and evaluates treatment plans * Prescribes and recommends pharmacological and non-pharmacological interventions * Initiates appropriate specialty consultations referrals when requisite * Initiates and evaluates clinical pathways and protocols * Collaborates with Attendings and/or Fellows regarding admissions and community transfers * Facilitates the timely flow of patients via collaboration with interdisciplinary team members and supports all interdisciplinary relationships * Recognizes acute/emergent situations requiring immediate responsiveness and initiates life-saving interventions, alerting the interdisciplinary team members for assistance * Recognizes and incorporates age-specific requirements, communicates plan of care with patients and families * Advocates for patients and family members in educational needs, treatment options, participation in care and ethical issues and patient rights with recognition and consideration of the diverse needs of the individual/family * Provides consultation in specialty area * Maintains confidentiality of patient information * Coordinates meeting applicable clinical standards for all disciplines in collaboration with interdisciplinary team members * Explains the necessity, preparation, nature and anticipated effects of procedure(s) to patients, patient families, staff and students * Performs designated procedures after demonstrated competency Educational: Leadership: Quality/Stewardship/Outcomes: Research: Professional Development: MINIMUM REQUIREMENTS Education: Master's Degree as an Advance Practice Provider - Nurse Practitioner or as an Advance Practice Provider - Physician Assistant Experience: One to two years minimum experience as an Advanced Practice Provider Nurse Practitioner or Physician Assistant preferred. Licensure: License or license eligible as an Advance Practice Provider (Nurse Practitioner or Physician Assistant) as set forth by the state of Virginia. If Nurse Practitioner, license to Practice as a Registered Nurse in the Commonwealth of Virginia is required. Must demonstrate and maintain all credentialing and licensing requirements of the UVA Medical Center, the Virginia Board of Medicine or the Virginia Board of Nursing, and the appropriate certifying bodies. Must be enrolled with all payers through Delegated Credentialing and can participate in Medicaid and Medicare for billing. Current Basic Life Support (BLS) required. Current PALS/NRP, as required specific to position. Advanced Cardiac Life Support (ACLS) as required. PHYSICAL DEMANDS Job requires standing for prolonged periods, frequently traveling/bending/stooping. Proficient communicative, auditory and visual skills; Attention to detail and ability to write legibly; Ability to lift/push/pull 20 - 50lbs. May be exposed to chemicals, blood/body fluids and infectious disease.
