Overview

As a member of the behavioral health care management team, this position integrates the care of the individual within Behavioral Health Services. These team members contribute professional and clinical knowledge and skills to support patient and family members access to behavioral health services. Those services will include, but are not limited to discharge placement referrals, interdepartmental referrals and reimbursement options for patients. The care coordinator serves as a central source of communication for the patient and their support systems. In addition to the coordination of care, this position provides assessment of psychosocial factors that influence a patient's health with connection to community resources to alleviate barriers. Works in the primary setting of the Emergency Department but can be askedto serve in various areas of the hospital setting providing direct care and emotional support, transition planning and facilitation, and advocacy.

Responsibilities

Patient Care

  • Performs psychosocial assessment of the patient to identify priority needs, strengths, patient preferences and barriers to care
  • Provides brief crisis intervention services and emotional support to patients/families to enhance the ability to cope with the impact of health conditions
  • Assesses grief issues and offers bereavement support
  • Assists with planning for care transitions and collaborates with UPH, community services, and facilities to support patient safety and continuity of care.
  • Documents assessments, interventions, and referrals in the electronic health record according to documentation standards.
  • Assists in identifying the correct level of care for patients and locating appropriate placement.

Care Coordination/ Discharge Planning

  • Identifies patient transitional needs by assessing psychosocial, environmental, financial and cultural strengths and barriers.
  • Maintains comprehensive knowledge of community resources and acts as a liaison to refer patients/families to health and social services, health insurance, public assistance and other resources to meet patient identified needs. *
  • Provides expertise and plays a key role with the care team in establishing patient-centered goals of care and identifying psychosocial and behavioral strengths and barriers. *
  • Contributes to the comprehensive, longitudinal plan of care based on patient-centric goals and coping strategies. *
  • Facilitates and/or participates in interdisciplinary team meetings to review and revise the patient plan of care. *
  • Collaborates with social workers and other professionals across the continuum and in the community to ensure continuity of care. *
  • Assumes accountability for the development and implementation of an effective discharge plan for complex-care patients. Works with internal and external resources to coordinate a timely and safe transition of patient to the appropriate level of care. *
  • Documents the case management plan to include: clinical needs, barriers to quality care, effective utilization of resources and pursues referrals in timely manner.

Basic UPH Performance Criteria

  • Demonstrates ability to meet business needs of department with regular, reliable attendance.
  • Employee maintains current licenses and/or certifications required for the position.
  • Practices and reflects knowledge of HIPAA, TJC, DNV, OSHA and other federal/state regulatory agencies guiding healthcare.
  • Completes all annual education and competency requirements within the calendar year.
  • Is knowledgeable of hospital and department compliance requirements for federally funded healthcare programs (e.g. Medicare and Medicaid) regarding fraud, waste and abuse. Brings any questions or concerns regarding compliance to the immediate attention of hospital administrative staff. Takes appropriate action on concerns reported by department staff related to compliance.

Qualifications

Education: Master Degree required in Social Work, Professional Counseling or Marriage and Family Therapy with a clinical/mental health concentration from an accredited school.

Experience: Applicants should have completed practicum experiences as part of their accredited program. Emergency Department or Integrated Mental Health Clinic experience.

License(s)/Certification(s): Iowa: LMSW, tLMFT, tLMHC. Illinois: LPC or the LMSW (temporary licensure is not a requirement if applicant meets QMHP criteria). Wisconsin: CAPSW, LPC-IT, MFT-IT. May be required to obtain BLS within six months of hire for certain sites. Iowa: LISW, LMFT, LMHC. Illinois: LCPC, LISW, LCSW, LMFT. Wisconsin: LCSW, LMFT, LPC.

Knowledge/Skills/Abilities: Writes, reads, comprehends, and speaks fluent English. Basic computer knowledge using word processing, spreadsheet, email and web browser.
