Job Summary Responsible for supervising the condition management programs in order to improve the quality and cost effectiveness of care for patients with chronic conditions. Ensures the effective implementation of policies and procedures in compliance with regulatory requirements. Supervises the day-to-day operations of the program and its staff. Provides supervision and support of business health activities and staff that promote health and wellness services to modify/prevent behavior and lifestyle choices associated with chronic disease. In addition, provides support with program development and special projects.
Essential Functions

Manages the day-to-day operations of the organization's condition management program and staff, including implementation plans for internal and external clients. Collaborates with care management in order to provide coordinated service across the care continuum. Supervises health coaches to ensure proper development of patient care plans and ensures compliance with all required activities, established timeframes and mandatory reporting. Responsible for the recruitment, retention, staff competency, performance, evaluation, corrective action, career development and job satisfaction for condition management employees. Collaborates with the Manager of Condition Management in order to implement and coordinate activities that support accomplishment of program goals including, resource management, organizing, accessing, integrating and modifying work processes. Collaborates with Business Intelligence team to identify patients for condition management programs based on established identification criteria. Assist with monitoring program benchmarks, methodology for measuring condition-specific outcomes and overall program performance. Collaborates with multi-disciplinary team to develop and implement evidence-based population health strategies in order to improve the quality and cost-effectiveness of care for patients with chronic health conditions. Provides support with program development and special projects. Performs other duties as assigned.
Supervisory/Management Responsibilities
Job has direct and/or indirect supervision of team members that may include final budget authority, hire/termination authority, performance appraisal responsibility and disciplinary authority. Job will be considered a member of management staff at Prisma Health or affiliate and will have direct reports.
Minimum Requirements
Education - Bachelor's degree in Health, Wellness or Public Health Experience - Two (2) years in Health Education or Coaching
