Terros Health is pleased to share an exciting and rewarding opportunity for a MAT Case Manager working at our Stapley Dr. location in Mesa, AZ. Reporting to the Health Center Director, working with individuals diagnosed with a General Mental Health diagnoses. Embraces the recovery philosophy and promotes a safe recovery environment for clients. Uses brief clinical interventions, motivational interviewing, and supportive listening skills to engage and support clients receiving services. Our programs are accessible and focus on the whole person. Our programs include primary care, nutrition, counseling and groups sessions, addiction treatment, children's services, housing, and pharmacies. Sign-On Bonus: $2,000-External Candidates Only Full-Time, Employed Offering Excellent Competitive Compensation Monday - Friday No Weekends Great benefits, Including 401K and Generous PTO/PST (4+ weeks) Position Summary: Under the Agency's policies and professional requirements, participates as a member of an interdisciplinary clinical team working with individuals diagnosed with an opiate use diagnosis enrolled in medication assistance treatment. Manages those identified clients who receive MAT services. Completes assessments, crisis plans, and individualized service plans. Communicates and documents client progress toward their recovery. Embraces the recovery philosophy and promotes a safe recovery environment for clients. Uses brief clinical interventions, motivational Interviewing, and supportive listening skills to engage and support clients receiving services. Works under the direct supervision of the Regional Administrator or Clinical Manager. Essential Duties:
