Posting Summary |
Rutgers, The State University of New Jersey, is seeking a Patient Navigator for
NJMS- Infectious Diseases. The primary purpose of the Patient Navigator position is to provide clinic and community-based
HIV,
STI testing, and case management services to persons with and at risk for
HIV infection. Performs a biopsychosocial assessment and develops an informed care plan to address any identified social service needs and treatment adherence barriers. As part of the team, the Patient Navigator will provide services in clinical settings and on the mobile health unit including working select weekend and evening hours. Maintains the confidential integrity of all client level information. Proactively reports any issues requiring action, intervention, or resolution to the Program Manager in a timelymanner
Among the key duties of this position are the following:
- Provides a confidential assessment for those clients living with HIV and clients being referred for PrEP services determine the HIV prevention needs of the clients whether their test results are positive or negative; this includes the registration and planning of prevention services.
- Provides HIV and STI confidential pre and post-test counseling for identified patients and others according to NJ Department of Health and Senior Services and Centers for Disease Control and Prevention guidelines and documents accordingly
- Coordinates PrEP and PEP for clients
- Provides medical case management for newly diagnosed persons with HIV infection
- Maintains documentation of all client encounters and complete reporting requirements according to grant and department standards
- Performs phlebotomy as needed to help with medical follow up.
- Drives mobile health unit as needed. 8. Participates as an active member on committees as assigned

Qualifications
Minimum Education and Experience |
- Bachelor's Degree in Social Work or Human Services plus two (2) years (five (5) years preferred) of full-time professional experience with HIV/AIDS.
- Equivalent education, experience and/or training may be substituted for the degree requirement.

Certifications/Licenses |
- NJ certification as a HIV Counselor and phlebotomy certification required or must be prepared to obtain within six (6) months of employment. New Jersey Driver's License.
- Valid Driver's license required.

Required Knowledge, Skills, and Abilities |
- Must be able/willing to learn to drive mobile unit.
- Must have completed or be eligible to successfully complete the Rutgers Defensive Driving Training Course.
- Proficiency in Microsoft Windows based Excel, Access and Word required.
- Bilingual skills. (English/Spanish) preferred.
- Ability to exercise considerable independent judgment utilizing professional level of knowledge to establish the work priorities within the scope of responsibility.
- Excellent and demonstrated customer service skills required.
- Ability to work varied schedules which may include weekends/ evenings.
- Incumbent must fulfill all required job qualifications including requirements resulting from current/future modifications to federal/state funding guidelines.

PHYSICAL DEMANDS: Time spent on the physical activities: up to 1/3 standing, walking, sitting; over 2/3 using hand to finger, handle, or feel; over 2/3 reaching with hands and arms; no climbing or balancing; no stooping, kneeling, crouching, or crawling; over 2/3 talking or hearing, and no tasting or smelling. Position requires lifting up to 10 lbs. No special vision requirements.
