DESCRIPTION OF WORK

The Aging Care Manager 2 works with older adults from initial contact with individuals, their families, and professionals through ongoing care management responsibilities. Work includes specialized duties, (Protective Services, Care Transitions, Person-Center Counseling). A key responsibility will be maintaining a caseload of consumers and being involved in necessary areas of assistance. You may regularly coordinate services with physicians, community agencies, and financial institutions. Other duties to include but not limited to managing multiple tasks, paying close attention to detail, completing paperwork work assignments in a timely manner, as well as meeting deadlines.
Work Schedule and Additional Information:
- Full-time employment.
- Work hours are 8:00 AM to 4:30 PM, Monday - Friday, with a 60-minute lunch.
- On-call hours when required.
- Salary: In some cases, the starting salary may be non-negotiable.

REQUIRED EXPERIENCE, TRAINING & ELIGIBILITY

QUALIFICATIONS Minimum Experience and Training Requirements:
- Successful completion of the County Social Casework Intern program; or
- Six months as an Aging Care Manager 1 or a County Caseworker 1; or
- Six months of professional human services or nursing experience; and a bachelor's degree in social sciences, behavioral sciences, human services, or a closely related field; or
- An equivalent combination of experience and training.

- Hiring preference for this vacancy may be given to candidates who live within Lackawanna, Monroe, Pike, Susquehanna, or Wayne counties. If no eligible candidates who live within these counties apply for this position, candidates who reside in other counties may be considered.
- You must be able to perform essential job functions.
Legal Requirements:
- A conditional offer of employment will require a drug screening.
- You must pass a background investigation.
- This position falls under the provisions of the Child Protective Services Law.
- Under the Law, a conditional offer of employment will require submission and approval of satisfactory criminal history reports including, but not limited to, PA State Police clearance, PA Child Abuse history clearance, and FBI Fingerprint clearance.
