Job Summary

The Clinical Outcomes Nurse Educator will provide quality surveillance through event identification, investigation, reporting and follow up with loop closure for patients Grady Health System patients. Collaborates with Educators, Clinical Managers, patient care staff, providers, and the multi-disciplinary team to provide a continuum of care and promote optimal patient outcomes. Assists in coordinating routine activities associated with Grady health System including data gathering, data abstraction, data analysis, report producing, creating and sharing. To have a focus based on the achievement of hospital organizational quality goals to improve patient care. Strategizes with patient care leadership throughout Grady Health System and members of the Nursing Professional Development Department to meet both system and department-based goals.

Job Responsibilities

50% - Performance Improvement.

Identifies performance improvement opportunities using surveillance methods outlined by best practice models such as the Performance Improvement in Patient Safety (PIPS) plan, AHRQ, and IHI via quality assessment/audits and through communication with clinical staff.

Incorporate information from the Performance Improvement (PI) process into the Nursing Quality and Safety Huddles, presents cases at meetings pertinent to the same including Nursing Quality Committees, RCA's, event review, Patient Safety Medication Errors, Nursing Peer Review and other quality/performance improvement meetings.

Exhibits proficiency in report running and writing to support Patient Care Services Performance improvement efforts. Acts as a liaison and facilitates multidisciplinary and interdepartmental collaboration and communication between Nursing Professional Development and all patient care areas and providers. Applies expertise in nursing practice to facilitate patient care as needed.

25% - Professional Development and Continuing Education.

Serves as a mentor or coach to facilitate and support learning needs for Patient Care Services experienced nurses, Nurse Residents, Nurse Externs, and unlicensed assistive personnel. Consistently exhibiting capacity to communicate effectively with the team to influence change.

Exhibits high level of emotional intelligence. Takes an active responsibility for own learning in alignment with self0identified and leader-identified learning needs, and routinely participates in monthly/annual educational offerings. Maintains required Continuing Education units annually for NPD certification.

20% - Data Management.

Completes concurrent and retrospective data abstraction for chart reviews. To become and remain in compliance with routine data submission in Sentact reporting system.

Supports data validation efforts including supporting education with all quality initiatives such as Falls, CLABSI, CAUTI, HAPI, Sepsis, Restraints, HAI, VTE prophylaxis, and any other quality initiative that Grady Health System supports to improve patient outcomes. Identifies data inconsistencies, develops methods and leads team to correct/mitigate.

5% - Other Duties as assigned.

Performs other duties as prescribed by NPD Leadership to support the goals and mission of the Nursing Professional Development department and Grady Health System.

Education

Bachelor's degree in nursing.

Experience

6-8 years experience in related field.

Excellent written/verbal communication skills with a focus on developing and maintaining positive, collaborative relationships.

Strong analytical and problem-solving skills.

Motivated to acquire new skills and knowledge while working under pressure.

Knowledge of medical terminology and navigation of the Electronic Medical Record.

Demonstrates characteristics of being a change agent.

Skills/Certifications/License

Georgia License as a Registered Professional Nurse.

Basic Life Support (BLS) Certification.

ACLS Certification required for roles in all of the following areas within 90 days of employment:

All Critical Care

Emergency Care Center (ECC) GI/Cath Laboratory

Labor & Delivery

All Intermediate Care

Post-Acute Care Unit (PACU)

CEN/TCRN or CCRN within 2 years of hire for ECC-specific role.

TNCC or ATCN completion within 18 months of hire date for ECC-specific role.

NPD Certification (Nursing Professional Development) within 18 months of hire date.
