SUMMARY OF POSITION Be a critical part of Vibrant Health's patient centered medical home by assistant patients during the intake part of appointments. Rooms patients; checks vitals such as weight, height, blood pressure, temperature, measurements and more; ask screening questions for required programs; take notes about patient questions and issues for provider and other duties to help assess and provide excellent patient care. Ensures efficient patient flow throughout the clinic, maintains neat, clean, and safe patient care areas, and promotes patient satisfaction through professional and respectful communications. Works with health care teams to provide patient care that follows the Patient-Centered Medical Home model. ESSENTIAL JOB DUTIES
