Job Description
Payrate $20/hour Shift Details Monday-Friday 8:30am-4:30pm POSITION SUMMARY: Under the direction of the Executive Director, the Activities Coordinator will develop and train staff to implement activities for those individuals enrolled in Elwyn Adult Day Services to help develop an everyday life for each participant, giving each one an increased quality of life through community and facility based activities that strengthen the participant's abilities. DUTIES AND RESPONSIBILITIES:
- Assists the program staff at each assigned program to plan activities:
- Using a professional understanding of the needs and abilities of the participants, including activities reflecting awareness of functional level and emphasizing strengths and abilities in order for the individual participant to be able to have a feeling of accomplishment. This includes modifying tasks to accommodate needs.
- That are based on interests of the individual making them meaningful to the individual
- With multiple options for activities that can be tailored to the individuals to allow them to be able to participate with others in the same activity at their optimal level of functioning and to progress according to their own pace
- That are designed to promote personal growth and enhance self-image of the participants; to improve or maintain the functioning level of individual participants; or both
- Taking into consideration individual differences such as health status, lifestyle, ethnicity, religious affiliation, values, experiences, needs, abilities and skills by providing opportunities for different types and levels of involvement that include individual, small and large groups, and intergenerational groups
- At community venues, including both indoor and outdoor events, as appropriate
- Ensures that services and activities are implemented following all health and safety measures as indicated by each participant's medical history and program guidelines
- Meets with the program participants to gauge satisfaction with services and to gather ideas for future activities
- Attends Individual Support Plan (ISP) and Individual Program Plan (IPP) meetings as needed and available to support development of individualized activities in line with the needs identified
- Provides resources to increase the capacity of the Program Specialist to implement and modify activities to meet the needs of the participants including but not limited to building the skills of the individuals, making modifications, providing accessibility to activities, and using the functional assessment to personalize activities to assist in developing outcomes/goals based on individual skill building
- Develops the weekly program schedule with the program's director and Program Manager
- Develops the program curriculum in conjunction with the program's director and Program Manager in accordance with established guidelines to support activities developed
- Assists the Program Specialist in training and supporting the Day Program Instructors in the implementation and modification of tasks in an activity using the program curriculum
- Assists with the implementation of services and activities, for both community and facility based events
- Meets with Adult Day Program leadership as scheduled to discuss and assess participant process and progress with activities and to determine the need to replace or develop new activities for the program
- Monitors activities and provides feedback to Day Program Instructors on the implementation of the activity using established feedback form
- Completes and submits monthly reports to the director of the program summarizing assessments completed and need for development or correction to any individual's plan to ensure participant success
- Researches and procures activity and service supplies as requested
- In concert with the Executive Director and Program Directors, seeks out resources and makes contacts with community members and local businesses, school or clubs to secure partnerships to assist with activities that are accessible and meaningful to the participant within the community.
- Provide back-up to the Program Manager and/or Program Specialist as needed
- Perform cardiopulmonary resuscitation (CPR), and crisis intervention using agency-trained protocols
- Perform other duties as assigned
IMMEDIATE SUPERVISOR: Executive Director, Adult Day Programs DIRECT REPORTS: None CONTACTS: Individuals supported/enrolled in Elwyn Day Services, Elwyn staff, families, funding sources, outside agencies, businesses, schools, clubs, and the community EDUCATION/EXPERIENCE/SKILLS REQUIREMENTS:
- Associate Degree in Human Services with four (4) years of previous experience working with OR a Bachelor's Degree in Human Services with two (2) years of previous experience working with people with disabilities OR a Master's degree or above from an accredited college or university with 1 year of work experience working directly with people with disabilities
- Established community contacts and network for resources for activities
- Demonstrated ability to work effectively as part of a team
- Ability to work in a fast-paced environment and manage and prioritize multiple, often competing, priorities
- Demonstrated strong attention to detail
- Demonstrated strong time management and organizational skills
- Demonstrated excellent judgment with the ability to independently solve problems and make decisions with little or no need for direct supervision
- Must be able to perform CPR and crisis intervention using agency-trained protocols
- Must possess excellent customer interaction, collaboration, presentation, and written and verbal communication skills
- Demonstrated intermediate experience with Microsoft Office applications, including Word, Excel, Outlook, and PowerPoint
- Successful completion of ACRE training within 3 months of hire
- Must have current, valid driver's license in state of residence, three (3) years driving experience in the United States, and acceptable driving record
