The Supervisor, Population Health ACO Care Coordinator will be responsible for supporting the day to day function of the Care Coordination Team. Responsibilities for this role include, but are not limited to: Organizing workflows, ensuring that Team Members understand their duties or delegated tasks. Conduct Training. Monitor productivity reports. Coaching Team Members. Complete yearly reviews. Disciplinary Actions. Act as a Practice Liaison. This reposition reports to the ACO Administrative Director or Manager of ACO Care Coordination. In this position, you will need to be in the office every week for 4 days. Travel is required to the Neptune City office and to physician practices from the Riverview Medical Center area to the Southern Ocean Medical Center area. Sometimes, you will need to travel to hospital sites for the entire Southern Region.
