Ambulatory Care Specialty Clinical Pharmacist

The Ambulatory Care Specialty Clinical Pharmacist (ACSCP) plays a crucial role in enhancing patient care within the Medication Management Clinic (MMC). Under the guidance and direct supervision of the Pharmacy Director and the general supervision of the program Medical Director, the ACSCP collaborates with clinicians and caregivers to provide coordinated, innovative, comprehensive, high-quality care to patients referred to the Medication Management Clinic (MMC) for focused drug therapy management of acute and chronic disorders. The ACSCP will be responsible for the promotion of rational, safe, and cost-effective drug therapy in collaboration with internal primary care clinicians and specialists, nurse care managers, our community health partners, and other clinical associates. The ACSCP is responsible for providing collaborative drug therapy management in the clinic (face-to face), telephonically, and via telehealth. In addition, the ACSCP will assess the patient, initiate and adjust medications, screen for and minimize drug interactions and adverse effects, provide disease/medication education, and explore opportunities to simplify and/or decrease the cost of the medication regimen in collaboration with the referring clinician, patient and/or their caregiver, and other team members. In addition, he/she will develop and sustain productive patient relationships to ensure integrated and comprehensive care. These services are provided within the framework of a pharmacy-clinician collaborative practice agreement. Other job responsibilities include, but are not limited to:

* Communicate clearly (oral & written) and openly with patients and other healthcare professionals.

* Able to work productively with other members of the patient care team.

.Precepting of residents (PGY1/PGY2) and students as needed.

* Participation in pharmacy and organization committees and shared governance.

* Contributes to process improvement projects and patient experience.

* Provide drug information and education to medical staff and patients.

* Build relationships to promote a collaborative environment.

* Be accountable for your performance at the MMC.

* Take initiative for your professional growth.

* Maintains accurate electronic health records of patients and completes documentation in a timely manner.

* Performs other related duties as assigned.

Located just minutes from the shores of Padre Island, CHRISTUS Spohn Hospital Corpus Christi-Southprovides surgical and medical services for our patients in south Corpus Christi. Perfectly positioned to serve the rapidly-growing south side of Corpus Christi, CHRISTUS Spohn Hospital Corpus Christi - South is licensed for 158 beds. South offers a full range of medical and surgical services including pediatrics, obstetrics, skilled nursing, emergency, intensive care as well as advanced diagnostic and imaging. The hospital is also home to a Level III Neonatal Intensive Care Unit which enables recovering mothers to remain close to their premature or special needs babies.

Requirements:

Education/Skills


  • Doctor of Pharmacy Degree from an ACPE accredited school of Pharmacy is required

Experience


  • Completion of a PGY1 residency or 2-year of clinical practice (i.e. ambulatory care, community, or hospital) is required.
  • PGY2 residency in Ambulatory Care Pharmacy is preferred.

Licenses, Registrations, or Certifications

* Current, valid Texas Board of Pharmacy License

* Must obtain Board Certified Pharmacotherapy Specialist (BCPS) or Board-Certified Ambulatory Care Pharmacist (BCACP) prior to orwithin two (2) years of hire as appropriate
