Pharmacist- Clinical Specialist/Oncology

Job Summary:

Clinical Pharmacy Specialists are responsible and accountable for the provision of safe, effective, and prompt medication therapy within a clinical area. They are responsible for the monitoring and management of medication regimens either in consult, per protocol or as a recommendation to providers.

Principal Duties and Responsibilities:

The following is a summary of the major functions of this individual's job. He or she may perform other duties which are not mentioned below, and specific functions may change from time to time.

  • Works collaboratively with physicians and other members of the healthcare team to develop programs/guidelines that improve practice and patient outcomes.
  • Demonstrates an understanding of oncology disease states and treatments.
  • Adjusts doses of chemotherapeutic drugs based on changes in patient BSA, renal function, complete blood count or other clinical situations
  • Provides patient education regarding chemotherapeutic regimens.
  • Provide guidance and appropriate utilization of hemostatic agents, hazardous agents, novel therapeutics, and other high-cost/high-risk agents as needed.
  • Provide assistance with multidisciplinary teams for palliative care/end of life care
  • Maintains knowledge of local, state and federal regulations regarding pharmacy practice as well as Accreditation Commission for Healthcare (ACHC) standards. Works to ensure compliance with these regulations. Performs Medication Area Inspections as assigned, contacting both Pharmacy Manager and Patient Care Unit's Manager regarding issues noted.

Minimum Education, Training and Experience Required:

A. Graduate of an accredited College of Pharmacy with Pharm D in Pharmacy.

B. Kentucky Pharmacist Registration or eligible.

C. Residency training or previous oncology experience required.

D PGY2 or 5 years direct oncology experience in lieu of required

E. Board Certified Oncology Pharmacist (BCOP) preferred (goal is to obtain within 2 years of hire if does not have)
