Job Description: Pharmacist- Clinical Specialist/Oncology Job Summary: Clinical Pharmacy Specialists are responsible and accountable for the provision of safe, effective, and prompt medication therapy within a clinical area. They are responsible for the monitoring and management of medication regimens either in consult, per protocol or as a recommendation to providers. Principal Duties and Responsibilities: The following is a summary of the major functions of this individual's job. He or she may perform other duties which are not mentioned below, and specific functions may change from time to time.
Minimum Education, Training and Experience Required: A. Graduate of an accredited College of Pharmacy with Pharm D in Pharmacy. B. Kentucky Pharmacist Registration or eligible. C. Residency training or previous oncology experience required. D PGY2 or 5 years direct oncology experience in lieu of required E. Board Certified Oncology Pharmacist (BCOP) preferred (goal is to obtain within 2 years of hire if does not have)
