Full-time

Via Linda Behavioral Hospital, newly opened, is excited to begin offering a full continuum of inpatient and outpatient services to the Scottsdale, Arizona population and the greater Phoenix region. Our 120-bed facility offers specialized mental health services and substance use treatment for teens, adults and older adults experiencing issues such as depression, anxiety, personality disorders and co-occurring addictions.

Position Summary:

The Step-Down Navigator is responsible for meeting with all eligible patients to discuss the benefits of Outpatient services as part of their continuum of care treatment. The Step-Down Navigator will be responsible for communicating eligibility to key members of senior leadership & associated departments and will be responsible for routinely collecting and entering data, specific to Outpatient eligibility, into Midas for trending report purposes. They will also complete appropriate documentation in MS4 for inquiry calls for Outpatient. They will also assist with outcome reporting as needed. They will have a thorough understanding of the Step-Down Playbook, implementing components of the Playbook as needed. A thorough understanding of the Inpatient workflow process, as it pertains to case management and discharge planning, is preferred.


Qualifications

Position Qualifications:

  • Preferred Education: Master's Degree from an accredited college or university in counseling or related field.
  • Minimum Experience: A minimum of Two (2) years' experience in a psychiatric/mental health field, preferred.
  • Required Certification and License: LMSW, LCSW, LMFT, LPC, LAC
  • Must have or be able to obtain a Level One Fingerprint Clearance Card
